As the Registered Nurse - Home Care Case Manager working in ED to Home role with the Intensive Home Care Team, you will have the opportunity to work within a uniquely diverse environment and enjoy the benefits of a larger team while collaborating with other professionals. As part of a team of Registered Nurses and Social Workers, you will support clients within the Emergency Department to facilitate access to community resources and enhance care through effective communication with the care team. You will support client transitions across the care spectrum. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
