We are seeking a dedicated and experienced MDS-RAI and In-Service Coordinator to join our healthcare team. The ideal candidate will be responsible for coordinating the assessment and care planning processes for patients, ensuring compliance with regulatory requirements, and enhancing the quality of care provided. This role requires strong clinical knowledge and excellent communication skills to effectively collaborate with interdisciplinary teams.
Duties
Conduct comprehensive assessments of residents to determine their needs and develop individualized care plans.
Ensure accurate completion of Minimum Data Set (MDS) assessments in accordance with federal and provincial regulations.
Collaborate with nursing staff, physicians, and other healthcare professionals to coordinate resident care.
Monitor resident progress and adjust care plans as necessary to meet changing needs.
Maintain up-to-date knowledge of regulations related to MDS and participate in quality improvement initiatives.
Provide education and support to staff regarding MDS processes and documentation requirements.
Co-ordinate annual education for staff and maintain on-line learning system (SURGE)
Assist in case management activities, including pain management, home care coordination, and transitions of care.
Document all relevant patient information accurately in electronic health records (PCC).
Engage with families and caregivers to ensure understanding of care plans and address any concerns.
Requirements
Proven experience in nursing or healthcare coordination, preferably in RAI-MDS and LTC Settings
Familiarity with medical terminology, medication administration, phlebotomy, and laboratory procedures.
Experience working with diverse populations including pediatrics, seniors, and individuals with dementia or chronic conditions.
Strong understanding of case management principles and pain management strategies.
Previous experience in LTC and Unionized environments is highly desirable.
Excellent communication skills with the ability to work collaboratively within a multidisciplinary team.
Proficient in using electronic health record systems especially Point Click Care.
A valid nursing license is mandatory; relevant certifications in MDS coordination are advantageous.
