The RAI Coordinator plays a key leadership role in ensuring the accuracy, quality, and compliance of Resident Assessment Instrument (RAI-MDS) data. This position works closely with interdisciplinary team members to support resident-centered care planning, optimize funding, and maintain regulatory standards within the home.
Key Responsibilities
Lead and oversee the RAI-MDS assessment process, ensuring timely and accurate completion of all assessments
Validate assessment data and submissions to ensure compliance with Ministry of Long-Term Care (MLTC) and CIHI standards
Support clinical teams in developing, implementing, and evaluating individualized care plans
Conduct audits to identify documentation gaps, provide coaching, and enhance data quality
Collaborate with nursing leaders and interdisciplinary staff to maintain high standards of clinical documentation
Monitor and analyze RAI data trends to support quality improvement initiatives
Prepare and submit mandatory reports, including quarterly and annual submissions
Provide training and ongoing support to staff on RAI processes, documentation requirements, and care planning best practices
Participate in internal and external audits, inspections, and accreditation activities
Ensure compliance with Long-Term Care Homes Act, Fixing Long-Term Care Act, and all relevant regulations
Qualifications
Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the CNO
Minimum 2 years of experience in long-term care, with strong knowledge of RAI-MDS 2.0
Demonstrated expertise in clinical documentation, assessment, and care planning
Strong understanding of Ontario long-term care legislation and compliance requirements
Excellent analytical, organizational, and problem-solving skills
Proficiency with PointClickCare (PCC) and RAI-related software
Strong communication and collaboration skills
Ability to train, mentor, and support interdisciplinary team members
Preferred Assets
Previous experience as an RAI Coordinator or similar clinical leadership role
Knowledge of quality improvement processes
Experience working with CIHI submissions and funding methodologies
