Carefirst Seniors & Community Services Association
is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.
We are currently seeking a
Personal Support Worker (PSW) Supervisor
to join our Home Care team.

Responsibilities:
Provides client care needs assessment (including use of the InterRAI Cha), case management, crisis intervention, supervision and clinical field support for community health care providers delivering Home Care, and Assisted Living services
Supports the delivery of safe and effective services through ongoing supervision, performance management, and training to personal support workers/ home helpers/care attendants, in order to meet client-centered care needs, and agency contractual and quality improvement obligations as per legislative, agency and accreditation standards
Acts as a clinical consultant for the Home Care team/department, Assisted Living, and other medically complex cases as required
Prepare and submit Initial and Interim Client Assessment Reports to LHIN and/or other service partners as required
As a departmental and organizational team member, attend meetings and contribute to the planning, monitoring, reviewing, and evaluating of programs/services for continuous quality improvement, thus preparing progress and evaluation reports for the Program Director on a regular basis
Attend the organization's Infection Prevention and Control (IPAC) Committee and provide guidance to the organization related to outbreak and pandemic planning
Provide coverage for Program Manager's activities in lieu of vacation, illness, etc
Requirements:
Post-secondary degree or diploma in Nursing or Practical Nursing
3 years' experience in a healthcare or community-based setting
Member of College of Nurses of Ontario in good standing
Valid First Aid & CPR certification
Strong leadership and organizational abilities with clinical assessment skills and judgement in understanding clients' needs
Excellent communication and interpersonal skills with both internal team members and external contacts
Proficient computer skills in Microsoft Office, understanding of InterRAI Cha and CIMS is an asset
Ability to work independently with minimal direct supervision
Flexible to change in work assignments and schedule (including occasional evenings and weekends)
Proficiency in English and a second language reflecting the community such as Cantonese or Mandarin, is an asset
Valid driver's license and access to a reliable vehicle for work travels
Valid police vulnerable sector check completed within one year
