Position Summary:

The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN\xe2\x80\x99s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.

The Social Medicine Program is seeking a Project Coordinator who will be responsible for leading all aspects of the project life cycle, from planning to implementation and evaluation, working with the program manager to set project timelines, coordinate activities, and ensure the project remains on budget. The Project Coordinator supports and monitors project progress against metrics, timelines, and plans; actively identifies and resolves issues affecting timely and successful completion of projects, and escalates issues to the Manager of the Social Medicine and Population Health Program. The Project Coordinator will be responsible for executing the implementation of key project and program initiatives in social medicine, working in collaboration with UHN programs and community organizations.

Duties

  • Serves as the main point of contact for project implementation using best practices to achieve project goals, including obtaining local client, clinical, community, organizational, and system perspectives
  • Lead all aspects of the project life cycle, working closely with the Manager and key stakeholders to:
  • Advance health equity in project delivery
  • Identify key stakeholders
  • Develop project metrics, surveys, and data solutions
  • Ensure projects are completed within budget and specified timeline, and refine approaches as needed
  • Prepare reports and obtain stakeholder and project team feedback
  • Develops Standard Operating Procedures (SOPs) for implementations in order to ensure consistency, safety and effectiveness
  • Review data from various sources, analyze and interpret the information to inform project progress
  • Lead discussion on project progress and present to senior management on key milestones
  • Develops strong relationships with stakeholders and leverages multiple communication mechanisms and styles to effectively manage relationships with key stakeholders to ensure engagement in and continued support of projects
  • In collaboration with the evaluation specialist(s), works to ensure projects meet targeted and measurable outcomes
  • Identify key issues and potential risks to project and stakeholder relationships. Provide recommendations to the Manager and Executive Director related to issue resolution options
  • Manages multiple competing priorities, and able to rapidly shift focus in response to changing priorities
  • Strong knowledge and understanding of population health, health equity, and the social determinants; as well as health system design and implementation to address inequities
Qualifications
  • Completion of a bachelors degree or equivalent in health sciences, health studies, public health, social services, or recognized equivalent field
  • At minimum, 5 years practical and related experience, and/or on the job training
  • Project planning, management, and implementation experience required, particularly as it relates to the social determinants of health; certification in Project Management preferred
  • Strong experience building connections and relationships with community organizations
  • Strong knowledge of, and familiarity with, community resources to address the social determinants of health, including housing, poverty, and food insecurity
  • Dynamic knowledge of community engagement principles
  • Demonstrated commitment to and understanding of equity, anti-racism, and anti-oppression principles and practices
  • Effective communicator with strong organizational skills and demonstrated ability to foster relationships and partnerships with internal and external stakeholders
  • Understanding of acute care setting and ability to navigate hospital systems an asset
  • Knowledge of the Ontario and Toronto health care and social services systems and public health an asset
  • Ability to produce high quality reports and meet deadlines
  • Well-developed decision-making, problem-solving, and judgement skills, as well as excellent time management and organizational skills required
  • Excellent interpersonal, oral, and written communication skills
