Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityReporting to the Community Transition Manager, the Project Coordinator, Community Transitions will provide support to advance the strategic priorities in the Community Transitions portfolio. The position will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in this portfolio.The scope of practice for this position will include collaborative work with NSH programs & services, contracted health care providers and community members to bring together diverse perspectives and experiences, including people with lived and living experiences. They are responsible for supporting the work of Community Transitions through practice facilitation and practice support to help communities make measurable improvement during periods of transition. They will work closely with community transition team members to facilitate and assist with the measurement and evaluation of practice improvements and to share/promote the successes of these improvements internal and external to NSH. Using both qualitative and quantitative evaluation methods, the position will work collaboratively with staff, to gather and analyze relevant practice data to inform the decision making on proposed practice changes. The position will also aid in the development of policies and procedures relating to practice facilitation. The position serves as a resource to other team members in Community Transitions.This position will research, and work with others to determine best practices in areas of equity and access healthcare to vulnerable populations such as unhoused clients through partnership and stakeholder management in the community. Through relationship building, they will deliver transition services to vulnerable clients.
