Position: Program Lead, UHN at Home and Older Adult Strategy

Reports To: Director, UHN at Home
Salary Range: $83,187 - $103,974 per annum
Status: Permanent Full-Time, contingent on funding

UHN at Home is a new community-based program that leverages strategic partnerships with Service Provider Organizations (home care and other community care providers), and establishes a team of UHN staff who partner in the delivery of care in the community.

As an integral member of the UHN at Home Leadership Team, the Program Lead performs activities to facilitate the effective functioning of the UHN at Home team and Integrated Care.

Key Responsibilities

  • Lead the development and implementation of the Care Coordination model for UHN at Home and Integrated Care
  • Supports recruitment, training and ongoing mentorship and coaching for IC Leads and other relevant roles in the department, in collaboration with the Director
  • Performs regular duties of an Integrated Care Lead as required. This includes care coordination, navigation, patient care support, clinical coaching and more
  • Performs process improvement activities within the UHN at Home Program, including understanding possibilities of Integrated Care and UHN at Home and driving changes in models of care that utilize resources differently across the health system
  • Performs quality management responsibilities, including identifying and implementing new processes and procedures to improve program functioning
  • Performs consultation functions within the Leadership Team, including development and implementation of UHN at Home and the Older Adult strategies

  • Performs administrative functions in support of the program areas and the Leadership Team.
  • Develops, promotes and delivers education programs and services
  • Performs cross-functional and/or other duties consistent with the job classification, as assigned or requested.
  • Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention's of the Act, in a timely manner.
Education
  • At minimum, completion of a Bachelor's degree in a regulated health profession or recognized equivalent required
Qualifications
  • At minimum, over 4 years up to and including 6 years practical and related experience
  • Five (5) years relevant clinical experience in acute, home, and/or community care sectors
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand.
  • Effective organizational, interpersonal and communication skills.
  • Experience working with personnel at senior levels
  • Facilitative leadership style
  • Ability to teach and communicate expert job knowledge and leading academic practices
  • Excellent team building, management and strategic planning skills

  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards.
  • Compliance with confidentiality requirements under UHN Privacy Policy.
  • Knowledge of applicable legislative, UHN and/or departmental policies.
  • A valid driver's license, adequate insurance (personal automobile policy with a minimum $1 million liability limit) and use of a reliable vehicle that qualifies for physical damage coverage is required
  • Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
PREFERRED
  • Completion of a Master's Degree in a relevant program or discipline
  • Demonstrated experience and competence in case management
  • Demonstrated recent comprehensive knowledge of community resources and health care delivery systems in acute and community settings
  • Experience leading a team
  • Experience with teaching and training of staff
Professional Affiliations/Memberships:
  • Current membership in good standing with applicable/relevant regulatory health professional body in Ontario required
