Job Summary:

The CAPABLE Program Coordinator serves as the first point of contact for the day-to-day operations of the CAPABLE program. Reporting to the Senior Manager or delegate, this position also works closely with community partners to develop and enhance key stakeholder relationships.

Key Responsibilities:

  • Works closely with Strategic Growth & Partnerships (SG&P) Office and the Project Coordinator to ensure successful program implementation, operation and evaluation.
  • Facilitates implementation of CAPABLE program guidelines and ensure best practice standards are adhered to.
  • Responsible for the planning and scheduling of client service delivery with the service delivery team.
  • Supports the day to day coordination of activities including those that support contracted work for handyman jobs identified by the clinical team and clients.
  • Ensures appropriate payment of contracted workers following VON Accounts Payable and Receivable process and policies.
  • In consultation with the SG&P office and the Project Coordinator, conducts formal program assessments and assist in the timely preparation and submission of data and reports (e.g. funder reporting that includes financial and CAPABLE program outcome reporting).
  • Establishes support strategies to assist the program when not meeting key performance indicators.
  • Develops and coordinate the delivery of program training workshops for staff and volunteers.
  • Coordinates annual grant application process.
  • Fosters good working relationships with key stakeholders and community partners.
  • Assists with recruitment and recognition.
External and Internal Relationships:
  • Works closely with the Project Coordinator and the CAPABLE National Centre to ensure appropriate knowledge transfer to operational teams and fidelity to CAPABLE program.
  • Supports engagement and evaluation activities utilized by the program and project teams to obtain key insights into the community needs and barriers as it relates to the implementation and ongoing provision of the program.
  • Supports the education and promotion of the CAPABLE program with key community referring organizations and trusted partner stakeholders.
  • Works with VON Communication and Marketing teams to provide required information and consultation on creative program pieces and announcements.
Education, Designations and Experience:
  • Post Secondary Diploma in Health/Social Science discipline or the equivalent combination of qualifications/education.
  • 2-3 years of related experience in community and health-based care.
  • 1-2 years of experience in program planning and evaluation is an asset.
  • Experience in volunteer administration an asset
  • Experience or understanding of the not-for-profit healthcare environment is an asset.
Skill Requirements:
  • Demonstrated commitment to working in an environment with high confidentiality and discretion.
  • Demonstrated knowledge of the social and health care services network and community resources, as well as a proven ability to build strong relationships within the community.
  • Excellent oral and written communication skills.
  • Able to work both independently and within a team.
  • Strong organizational skills with an ability to prioritize, multi-task and handle competing deadlines.
  • Strong customer service skills.
  • Ability to problem solve and adjust to rapidly changing priorities in a deadline.
  • Proficiency in Windows OS and MS Office Suite programs.
  • Must demonstrate attention to detail.
Other:
  • Must have personal vehicle and possess both a current driver\'s license and proof of vehicle insurance. .
  • A current and original copy of a satisfactory Criminal Records Check is required.
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
  • Ability to speak French is an asset in French Designated areas.
