The Smiths Falls Nurse Practitioner-Led Clinic is looking for a nurse practitioner to join our team!



The SFNPLC provides primary health care services to the people of Smiths Falls, and surrounding areas of Lanark, Leeds, and Grenville.



The health care we provide is delivered by nurse practitioners and supported by allied health professionals such as registered nurses, social worker, pharmacist and dietitian, as well as administrative support.



The nurse practitioners at the clinic work to the full scope of practice according to the Ontario College of Nurses RN-EC.

NURSE PRACTITIONER



SUMMARY



The Smiths Falls Nurse Practitioner-Led Clinic provides exemplary comprehensive primary health care services to the residents of Smiths Falls and the surrounding counties of Leeds, Grenville, and Lanark.

The Nurse Practitioner works to the full scope of primary health care nurse practitioner care within an interdisciplinary team.

ACCOUNTABILITY



The Nurse Practitioner reports to and is responsible to the Nurse Practitioner-Leads. All clinical decision making outside the scope of the Nurse Practitioner must be done in consultation with the Smiths Falls NPLC consultant physician and/or medical directives. The Nurse Practitioner must comply with the standards of practice guidelines of the College of Nurses of Ontario for the RN (EC).

ROLES AND RESPONSIBILITIES

:

Health Assessment and Diagnosis



Performs a comprehensive health or symptoms directed assessment and synthesizes data from multiple sources to make a diagnosis of a disease or disorder

Spends adequate time with the client to determine the presence of existing and potential health problems, with a major focus on related lifestyle, psychological, sociocultural, and environmental risk factors with may influence the client's health status, deals with these personally or by referral to or in consultation with the other team members or community programs or agencies

In accordance with the philosophy of primary health care, encourages clients and their families to take responsibility for their own health care by involving them in risk factor and health problem identification, goal setting and the choices of interventions for disease treatment and prevention and health promotion

Determines the need for and orders from the College of Nurses approved list and interprets the results of screening and diagnostic tests

Keeps complete, accurate, legible, and timely reports of client visits, using the charting format established by Smiths Falls Nurse Practitioner-Led Clinic, which will provide information to assist other practitioners in continuing the client's care

Sees clients at the Smiths Falls Nurse Practitioner-Led Clinic with scheduled and urgent appointments and in the community through outreach and home visits during regular and extended hours. Outreach includes community palliative clients and clients living in Long-Term
Care.

Therapeutics



Initiates and manages the care of clients with diseases or disorders within the Primary Health Care Nurse Practitioner scope of practice and/or monitors the ongoing therapy of clients with chronic stable illnesses by providing effective pharmacological, complementary, or counseling interventions

Prescribes medication as appropriate with the Primary Health Care Nurse Practitioner scope of practice including the use of medical directives

Health Promotion



Collaborates with community agencies to implement strategies to promote health and prevent disease with individuals, families, and groups

Family Health



Synthesizes information form individual clients to identify broader implications within the family

Assesses and manages the care of families in conjunction with community support agencies

Team and NPLC Responsibilities



Promotes and participates in an interdisciplinary provider model. Collaborates with both providers and clients to ensure that appropriate management, referral, and responses to inquiries are provided

Participates in the development of goals, and objectives of the Smiths Falls
Nurse Practitioner-Led Clinic related to quality improvement planning

Contributes to the development of program plans and develops, plans, implements, coordinates, and evaluates assigned projects in consultation with other team members

Supervises nurse practitioner students and participates in the education of other students

Contributes where necessary to program evaluation processes

Identifies supplies, equipment, and resources necessary to carry out programs

Performs other duties as assigned

The Physician/ Nurse Practitioner Relationship



The physician and the nurse practitioner will review any procedure beyond the scope of practice of the RN(EC) that is to be considered for inclusion into the role. If it is agreed the procedure is appropriate, then Medical Directives with appropriate processes will be developed. These will be reviewed annually.

A physician will be available for consultation via secure email.

The physician will participate with the NP in discussing therapeutic measures when necessary.

SPECIFICATIONS



Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent) Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred (continued) Must be a member in good standing with the Ontario College of Nurses as an RN(EC) Current Membership with NPAO and evidence of liability insurance Current Ontario Driver's License Ability to prioritize, manage time effectively and be flexible in a very active work environment Ability to operate with the highest level of confidentiality and discretion Ability to work both day and evening hours Ability to travel throughout Smiths Falls and the surrounding area for home visits Demonstrated computer skills Criminal record check required Nursing: 2 years (preferred) French speaking an asset
Job Types: Part-time, Permanent

Pay: $65.00 per hour

Expected hours: No less than 22.5 per week
