As a key member of Connexion Family Health Team (CFHT) and an integral part of an interdisciplinary health care team, you will deliver Nurse Practitioner (NP) care in accordance with Family Health Team policies, management instructions and professional practice standards. You will perform routine NP duties and ensure that the medical needs of our diverse patients are met in a caring, positive, team-oriented and patient-centred approach.
Your primary roles are to work in partnership with physicians and other health providers to perform assessments, provide treatment/management, education/advocacy and referrals & collaboration. You will also provide health education to our patients with a focus on health promotion, disease prevention and chronic disease management as well as lead and participate in programs currently being implemented in the Family Health Team.
You will be integrated into a multidisciplinary team wherein you will work with other health professionals providing the opportunity for our patients to have direct access to a broad range of primary health care services.
Roles and Responsibilities
Working within the scope of practice s/he performs comprehensive health or system directed assessment to make a diagnosis, as part of a health care team
Determine the need for orders and interpret the results of screening and diagnostic laboratory and imaging tests within the scope of practice
Provide advanced nursing care and treatment
Collaborate with physicians and other health care providers to develop and discuss appropriate care plan and individualized care management
Share education with patients by helping them to identify self-advocacy and the use of health resources
Document using patient health records, refer to and arrange follow-up as necessary
Develop and implement programs on health education, management and disease prevention
Skills, Knowledge Requirements
Strong verbal and written communication skills and the ability to adjust language and terminology to suit the individual patient or patient group
Excellent clinical assessment, treatment and management skills
Demonstrate cultural sensitivity and a commitment to the patient-centred care model
Autonomous and able to function with minimal supervision
Ability to work effectively with all members of the health care team
Commitment to work within a multi-disciplinary team environment
Ability to prioritize, manage time effectively and be flexible in a very active work environment
Demonstrate excellence in delivering education and teaching to a patient group setting
Experience with IUD insertion is preferred
Language Skills
Bilingual (French and English) for verbal and written communication is preferred
Education, Credentials and Qualifications
Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent)
Current registration and in good standing with the College of Nurses of Ontario
Current membership with RNAO and evidence of liability insurance required
Familiarity and experience in community and/or primary care setting
Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
Superior leadership, time management, communication and team-oriented spirit
Experience with an electronic medical record; PS Suite preferred
Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel and PowerPoint
Multi-tasking skills related to the delivery of the efficient primary care including the ability to share information and teach while treating a patient
Strong ability and commitment to work positively and productively within a multi-disciplinary team

Schedule:
Hours of Work - Monday to Friday 8:00 a.m. - 4:00 p.m., on-site, 37.5-hour work week with the possibility of one evening shift (11a-7p, Wednesdays) per week
