Position Summary:



The Nurse Practitioner functions as a member of the primary care team in the provision of primary health care services to residents of the community, with a strong emphasis on illness and injury prevention and health promotion.

Working with the Centre's priority populations, the Nurse Practitioner will provide primary health care services and participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.

The Nurse Practitioner serves as a champion of the Hamilton Urban Core's mission and vision and works in a manner that is consistent with the Centre's values, beliefs, core competencies and commitment to quality in service. The Nurse Practitioner is an essential member of the client-centered, inter-professional, multidisciplinary team of health providers at Hamilton Urban Core. The Nurse Practitioner will work with other members of the Centre's teams to effectively and efficiently serve clients and communities using responsive and innovative approaches to services and service delivery framed by health equity and the social determinants of health.

Accountability:



The Nurse Practitioner is a member of the Primary Health Care Team, is accountable to the team and reports to the Primary Health Care Manager. Overall accountability is to the Executive Director.

Key Responsibilities:



Perform assessments, diagnosis, treatment, health education/counseling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs). Work with the staff team and other service providers to coordinate client care including monitoring chronic conditions, responding to episodic illness, providing follow-up care, case management and referral. Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families, groups and the community. Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate charts, participating in chart reviews, case conferencing, case management and responding to client inquiries and referring individuals to appropriate treatment. Work with the health care team to plan and evaluate programs and develop or initiate health teaching, screening programs or workshops by identifying health needs of clients and various other community groups. Engage in outreach activities to facilitate access to primary health care including off-site visits and mobile services, where appropriate and approved. Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre's standards and commitment to learning Identify areas where the development of protocols and procedures need improvement, participate in committee work and staff meetings, maintain the laboratory and monitor controlled substances. Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses or educational leave). Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework Work as part of the interprofessional team at the Centre and adhere to the Centre's core competencies and the CHC Model of Care Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core

Qualifications:



BScN with Nurse Practitioner certificate and registered with the Ontario College of Nurses in Extended Class Broad-based nursing experience Excellent clinical, assessment and problem-solving skills Effective interpersonal and communication skills, and strong problem-solving skills Interest and experience in working in primary health care, preferably in a community health setting, and as part of a multi-disciplinary team Experience with and sensitivity to issues impacting on clients and communities accessing the Centre Experience with immigrants and refugees, seniors, youth, individuals or families with low/no income, people living with mental illness and/or people who are homeless or at risk of being homeless Experience working with refugees with complex clinical needs and require government assistance. Knowledge of the community health Centre sector is an asset.
