Job Description

We are seeking a dedicated and compassionate Nurse Practitioner to join our primary care team. The ideal candidate will have strong clinical skills, a passion for patient-centered care, and the ability to communicate effectively in English and Chinese/Mandarin. Responsibilities:

- Provide primary care to patients in a medical office and/or hospital setting

- Conduct comprehensive assessments of patients' health status

- Develop and implement individualized care plans

- Administer medications and treatments as prescribed

- Monitor and evaluate patient responses to interventions

- Collaborate with interdisciplinary healthcare team members to coordinate care

- Educate patients and families on disease management and preventive measures

- Perform case management duties, including referrals and follow-up care

Qualifications & Experience:

- Nurse Practitioner license in good standing

- Some experience as a Nurse Practitioner in a primary care setting

- Experience in home care, dementia care, or case management is highly desirable

- Proficiency in medication administration and documentation

- Strong assessment, critical thinking, and problem-solving skills

- Excellent communication and interpersonal abilities with patients, families, and team members

- Chinese/Mandarin speaking strongly preferred

Job Types: Full-time, Permanent

Pay: $75.00-$80.00 per hour

Expected hours: No less than 25 per week
