About the Opportunity
Reporting to the Primary Health Care Manager, the Primary Health Care Coordinator (Practice Support) will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator (Practice Support) role will have responsibility to lead the planning, ongoing development, implementation and evaluation of programs, services and project initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders across the Eastern Zone to ensure zonal support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The incumbent will support quality improvement initiatives for collaborative family practice and chronic disease management and wellness teams in primary health care.
The Primary Health Care Coordinator (Practice Support) will work collaboratively with family physicians and other health care providers and teams across the Eastern Zone to support quality improvement and change management strategies that are inherent in the services plan for primary health care.
