About the Opportunity
Reporting to the Network Manager of Primary Health Care the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The incumbent will support the Northern Zone\'s quality improvement initiatives for collaborative family practice and chronic disease management and wellness teams in primary health care.
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support change management strategies that are inherent in the services plan for primary health care.
