About the Opportunity
Reporting to the Primary Health Care Manager, the Primary Health Care (PHC) Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator role will be focused on supporting, reporting, optimization, efficiency and have responsibility to lead the planning, ongoing development, implementation and evaluation of services and project initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders across the Western Zone to ensure zonal and localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The PHC Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support quality improvement and change management strategies that are inherent in the services plan for primary health care.
