About Maple Family Health Team

Maple Family Health Team is a primary care organization that brings together physicians, nurses, and allied health professionals to provide and coordinate the best possible care for patients. At our Kingston clinic, you'll work with multiple physicians and care providers in a dynamic, inclusive, and patient-centered environment.

What You'll Do

As a

Registered Nurse

at Maple, you'll play an important role in supporting patients across the lifespan. Your work will include:

Providing care to patients of all ages--from prenatal to elder care Collaborating with an interdisciplinary team to plan, implement, and evaluate patient care Supporting Maple FHT programs and services Using evidence-based practice to deliver safe, effective care and patient education Participating in team meetings and ongoing education opportunities

What We're Looking For

Current unrestricted registration with the

College of Nurses of Ontario (CNO)

Experience in primary or community care (preferred) Knowledge of Kingston's community health resources Strong ability to assess and prioritize multiple patient and team needs Comfort with Electronic Medical Records (EMR) Experience supporting patients with conditions such as diabetes, lung disease, dementia, hypertension, and osteoporosis Training in smoking cessation counselling (asset) Commitment to maintaining confidentiality and patient trust

Position Details

Part-time, temporary

(3 days per week) Eligible for

HOOPP pension benefits

Vaccination requirements:

Full COVID-19 vaccination and proof of immunity to Measles Start date:

October 2025
