We are currently seeking: Pharmacists— Contract (Part-Time and Full-Time) for 1 year in Toronto, Candidates are expected to start in August (possibility of extension) for potentially the following projects, but not limited to: 1. Developing a new pharmacy role in the home care sector; this may include preparing research proposals, designing process maps, and meeting with external stakeholders. The pharmacist would also be expected to see clients in-person or virtually and ensure appropriate clinical functions are completed (e.g., patient counseling, appropriate documentation, pharmaceutical care, etc.) and work with the healthcare team to review medication use. 2. Ontario Health Team-related initiatives that require expertise from Pharmacists to contribute to advancing patient care in home and community care 3. COVID-19 and Flu Vaccination Efforts—handle, prepare and administer vaccines for injection; provide oversight and quality assurance to pharmacy technicians and/or nurses, manage inventory; provide patient counselling as appropriate; and complete all required reports and documentation. 4. Other VHA initiatives that would benefit from including Pharmacists as part of the care team Education and training will be provided with support in advance and on site to ensure all PPE is provided and physical distancing for those participating in the COVID-19 and/or flu vaccination efforts.
Required Skills

  • Graduate from an accredited post-secondary Pharmacy Program (i.e., B.Sc Phm, M. ScPhm, PharmD)
  • Successful completion of an OCP-approved injection training course is preferred; copy of certificate will be required as proof
  • COVID-19 vaccination is mandatory upon hire, vaccination receipt will be required as proof
  • Valid certification in CPR and First Aid is recommended
  • Strong leadership skills are an asset (i.e., strategic thinking, great team player, dependable)
  • Ability to integrate and apply critical thinking skills to client care and work independently
  • Committed to client centred care and client centred learning—with health teaching experience
  • Ability to work under minimal supervision and to manage competing priorities/deadlines
  • Ability to effectively present information and respond to questions from groups of managers, patients, nurses and physicians
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
  • Confident with Infection Prevention and Control practices and use of PPE
  • Experience in working with older adults with a commitment to cultural diversity
  • Previous experience with aseptic medication preparation and flu vaccine administration, an asset
  • Excellent communication and relationship building skills to enable collaboration with internal staff members and external partner organizations
  • Strong focus on social determinants of health and commitment to social justice and working successfully with marginalized groups
  • Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling
  • Valid driver’s license and access to a car is required to travel (preferred but not mandatory)
