Position Summary:

Reporting to the Clinical Manager of Pharmacy Services you will be an integral member of the Pharmacy team and be accountable for the pharmacotherapy for patients to ensure optimal patient care is achieved. You will be evaluating evidence-based medicine to support your recommendations and to develop improvement strategies for drug utilization.

Key Qualifications:

  • Baccalaureate Degree in Pharmacy or Pharm D
  • Licensed in Ontario to practice pharmacy
  • Completion of hospital pharmacy residency or equivalent experience is an asset
  • Strong interpersonal, problem solving, organizational, communication, documentation and critical thinking skills
  • Experience in providing exceptional patient centred care
  • Experience in hospital pharmacy systems and distribution systems are an asset.
  • Past experience or a keen interest in teaching and research activities is an asset
  • Relevant clinical experience preferred
  • Ability to work in a fast paced environment both independently and cooperatively with the team
  • Demonstrated self-directed learning and continuing education
  • Positive attitude, flexibility to adapt to changing organization needs and accountability
Responsibilities Include:
  • Provision of comprehensive pharmaceutical care to patients
  • Clinical care of patients in the pre-operative assessment clinic
  • Use of Pharmacists\xe2\x80\x99 clinical scope of practice as per THP\xe2\x80\x99s Policy and Procedure to optimize patient care
  • In collaboration with other clinical pharmacists deliver appropriate teaching to pharmacy residents and pharmacy students
  • Participatie in pharmacy case presentations, Pharmacy Education Rounds, journal clubs, case discussions and research projects
  • Drug distribution responsibilities.
  • Collaborate with physicians, nurses and allied health team members to provide optimized patient care
  • Other duties as assigned including the ability to potentially work at all Trillium Health Partners sites
