Your Opportunity:

The Crisis Stabilization Unit (CSU) is located inside the Emergency Department at Rockyview General Hospital. The CSU is a dynamic service made up of a patient and family centered multidisciplinary team who provide quality, client-focused, culturally appropriate, and integrated mental health services to patients on the CSU. The focus of the unit is on stabilizing acute addiction and mental health crisis, patient education, instilling hope in the patient's recovery, and connecting individuals to community-based resources within a 24-hour period. The Outreach Worker in consultation and communication with the CSU team will carry out treatment support functions and assist in facilitating linkages with community mental health services and multiple agencies immediately prior to and after discharge. This includes following up with patients after discharge from CSU and supporting clients to connect with community resources and support systems that enhance their functioning and living in the community of their choice. You will support patients in connecting with the Crisis Stabilization Unit Follow up clinic post discharge as outlined by the patients discharge plan. The Outreach Worker must be able to assess and intervene over the phone with patients in order to stabilize the patients in the community. In addition to phone calls, the CSU outreach worker visits clients in the community as needed. The successful applicant is an assertive, proactive individual, with a strong foundation in mental health, and possesses the ability to engage and work with a diverse client population. The successful applicant will demonstrate the ability to work in a collaborative, fast-paced, positive team environment that requires excellent observation, assessment, prioritization, conflict resolution, interpersonal, critical thinking and decision-making skills

Description:

As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Required Qualifications:

Completion of diploma from a post-secondary program in a related field. Additional Required Qualifications:

Completion of a diploma in a human service discipline (i.e., Social Work). Registration with the applicable regulatory body. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum 1-year experience providing outreach services to adults with mental health and addictions within the last 3 years. Knowledge of the addictions and mental health system, resources and the continuum of care within the Calgary zone. Knowledge of addictions, mental health diagnoses and suicide prevention. Verbal de-escalation skills. Preferred Qualifications:

Minimum 3 years' experience providing outreach services to adults with mental health and addictions within the last 5 years. Community-based mental health outreach experience. Group experience. Non-Violent Crisis Intervention within the last 2 years.
