Your Opportunity:

Skypointe 2 Complex Mental Health Rehabilitation Program is a unique new 32 bed Addiction and Mental Health Continuing Care unit located in the community of Skyview in Calgary opening November 2022. You will have the opportunity to work with a group of dynamic multidisciplinary professionals supporting clients in their rehabilitation and recovery journey in collaboration with our community partner AgeCare. The facility offers a woodworking shop, craft areas, library, group facilitation, quiet rooms and a fitness area to support the clients. Its rewarding being part of supporting individuals maintaining their psychiatric stability while assisting them to build skills required to reintegrate into the community. As the Outreach Worker you will work collaboratively with the interdisciplinary team, peace officers, as well as partnering agency staff members to provide care to clients both on site and in the community.The building is unique with multiple on site activities and programming available and you will provide support daily to assist clients in accessing the available on-site services and off-site as the clients prepare for transition. As the Outreach Worker you work as a member of a multidisciplinary team functions together to provide treatment, rehabilitation and support services and case management for our clients. Your role is to support the clients to meet the goals on their individual treatment plans. Tasks include supporting a client with their ADL's (activities of daily living) and IADL's (instrumental activities of daily living), participating in recreation, leisure, and therapy group/individual activities on site and in the community, supporting with bus training or driving clients to appointments. There are 2 positions available.

Description:

As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Required Qualifications: Completion of diploma from a post-secondary program in a related field. Additional Required Qualifications: Working knowledge of psychosocial rehabilitation, recovery, harm reduction model and current Mental Health Legislation. Awareness of the DSM-5 with respect to mental health diagnosis, symptomatology and interventions. Knowledge of relevant community resources and legislation. High degree of flexibility as well as the ability to work independently in an outreach capacity. Possession of a valid drivers license and access to a motor vehicle in good repair for work-related travel is require – that may include driving clients, if necessary. Preferred Qualifications: Experience with complex populations including the ability to work with potentially aggressive clients. Knowledge of relevant community resources and legislation. High degree of flexibility as well as the ability to work independently in an outreach capacity.
