Langs has grown to become a dynamic, respected multi-service organization that uses a community development approach to provide comprehensive health, social and recreational services that are responsive to the changing needs of our community.

What We are Looking For:



We are currently recruiting for an Outreach Worker to join our HART Hub team for a full-time position providing outreach support throughout the Cambridge and North Dumfries area. This role is responsible for ssupporting vulnerable and marginalized individuals by addressing basic needs and connecting them with health and social services, with a focus on housing, addictions, and the social determinants of health. They provide advocacy, build support networks, ensure warm referrals, and meet clients where they are.

A successful candidate in this role would have



Degree or diploma is social services or a related field Minimum of three years experience in social services, case management and service coordination with a focus on outreach Experience with advocacy Strong knowledge of community resources and services (Financial services, housing, etc.) Previous experience in non-profit sector an asset Strong written and verbal communication abilities Ability to manage multiple priorities and projects Excellent time management and organizational skills Experience navigating the housing systems an asset Demonstrated critical thinking and problem solving Flexibility and adaptability to changing priorities Detail-oriented with commitment to accuracy and quality Class G License and reliable access to a vehicle, travel is an expectation of this position

In this role you will be expected to complete the following tasks



Working in collaboration with the HART Hub and Interprofessional Primary Care (IPC) team, provide community resource and advocacy support to patients, with a focus on the social determinants of health/basic needs, addiction support, housing and access to services. Supporting and connecting with vulnerable populations (homeless or precariously housed) where they are at i.e. drop-in, encampments, homeless shelter Support the client with identifying social, physical, emotional and economic needs of patients Building on existing patient resources including family and friends, helps the patient to create an effective support network. Provide clear and timely communication with other healthcare and social service providers. Ensures warm transfers of patients between service providers and complete necessary referrals including attend appointments with patients Ability to travel in Waterloo Region and to work out of various primary care settings in Cambridge and North Dumfries Ability to work flexible hours, including one evening per week Completing on-going follow up with the patient and supporting with appointments and task completion Document and maintain clear records including supports, services and gaps as identified by the patient To complete other duties as required

The successful candidate would be eligible for the following benefits:



Starting Salary of $24.36-$27.35 Hourly 3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment (pro-rated to hours worked) 15 Sick/Personal Days per year (pro-rated to hours worked) 10 Professional Development days per year(pro-rated to hours worked) Enrolment in Healthcare of Ontario Pension Plan (HOOPP) Comprehensive Extended Health and Dental Care Plan including a $1000 Healthcare Spending Account.
