Outreach Workers are responsible for providing community support to clients of the Mental Health Program, which enables the client to maintain an optimal level of functioning while living with a chronic mental health illness. Outreach Workers work in an interdisciplinary team setting.


The Renfrew County Mesa HART Hub follows Mesa, a collaborative, multi-sector approach to providing compassionate care.


HART Hubs are designed to offer integrated services tailored to local community needs, focusing on individuals with complex service requirements, including those experiencing homelessness, substance use, and mental health challenges


These services aim to meet immediate needs and support long-term recovery and stability for individuals facing overlapping challenges.

REQUIREMENTS

Minimum two (2) year Post Secondary School Diploma in Social Service Work Previous experience working with individuals with severe mental illness in Renfrew County and in a community setting. Experience in a team-oriented environment an asset Valid G Driver's License (or equivalent) Has an available, safe vehicle, and clean driver's license record three (3) years prior to date of hire Carries appropriate automobile liability insurance of no less than $2,000,000 Demonstrated ability and experience facilitating life skills and social recreation groups Excellent interpersonal and conflict resolution skills Excellent verbal and written communication skills Knowledge of Renfrew County Excellent organizational skills and ability to set priorities Ability to work collaboratively as a team member Ability to coach, mentor and influence Staff may be cross-trained and support other function centres in a time of need Proficiency in the use of computer applications and software programs Embodies the LEAN ideology by actively and continually seeking opportunities for continuous improvement. Must have demonstrated ability to meet the attendance standards of the Hospital Bilingualism preferred (French/English)
