Occupational Therapist

Position Summary:
The Occupational Therapist (OT), Geriatric Medically Complex Care Team (GMCCT) will work within an interdisciplinary team to provide comprehensive care to older adults with complex and multi-faceted health needs. The role will focus on the community-dwelling geriatric population, with an emphasis on managing both acute and chronic health conditions and cognitive impairment. The OT will collaborate closely with other healthcare providers, including geriatricians, nurse practitioners, primary care physicians, nurses, pharmacists, and geriatric psychiatrists to support patients and their caregivers. The OT will utilize their advanced clinical skills and autonomy to assess, treat, case manage and promote wellness for patients in clinic and in community. They will also engage in program planning, quality improvement initiatives, and continuous professional development. Deliver education and resources to patients, families, and caregivers, focusing on self -management, and navigating the healthcare system.
Position Requirements:
Registered and in good standing with the College of Occupational Therapists of Ontario.
Graduate of an accredited Occupational Therapy Program.
Active Basic Cardiac Life Support (BCLS) certificate or ability to obtain within 3 months and maintain annually
Demonstrated ability to organize, prioritize, problem solve and respond to competing, unscheduled demands.
Demonstrated ability to function independently and as part of a multidisciplinary team.
Proven excellent interpersonal, written and verbal communication, organization, and time management skills.
Valid G driver's license and a reliable vehicle required
Demonstrated knowledge of Disability and Occupational Function issues in older adult population
Experienced in behavior care planning experience for individuals with dementia, delirium and Behavioral and Psychological Symptoms of Dementia
Demonstrated knowledge of the functional impact for individuals with a diagnosis of acute, chronic and palliative conditions.
Demonstrated ability to apply current knowledge, theory, evidence, and sound clinical judgment in patient management.
Knowledge of community support services and resources available.
Proven knowledge and experience with program development and evaluation.
Demonstrated commitment to maintaining and promoting healthy working relationships.
Demonstrated ability to apply best practice modalities for assessment and treatment within occupational therapy and professional scope of practice.
Demonstrated ability to work with diverse patients and staff population
Good attendance
Three (3) to five (5) years of work experience with a geriatric population
ADP Authorizer Status is an asset
Commitment to pursuing additional qualifications and skill development
Membership with Canadian Association of Occupational Therapists or Ontario Society of Occupational Therapists is preferred.
Knowledge of the community and resources available.
Certification in Gentle Persuasive Approach, MoCA, Motivational Interviewing or PIECES
