The Alex's Senior Health Centre provides comprehensive, team-based care tailored to adults aged 55 and older facing complex health and social challenges. The program offers medical services, mental health support, social work, and health navigation in one accessible location, helping seniors manage chronic conditions, maintain independence, and improve overall quality of life. With a focus on dignity, prevention, and collaboration, the centre supports aging in place and reduces barriers to care for vulnerable seniors in Calgary.

Reporting to the Program Manager, the Occupational Therapist is a key member of the Seniors Health Clinic interprofessional team and is responsible for assessing and supporting vulnerable older adults to optimize function, independence, safety, and participation in meaningful daily activities. The Occupational Therapist provides clinic-based care with home/community visits (as appropriate), delivers individual and group interventions, and collaborates closely with clients, caregivers, and the broader care team.

What You Will Do

Program PhilosophyOffer culturally safe, trauma-informed and age-friendly services, where indicated. Uphold person-centered, strengths-based, function-focused care within an integrated team-based primary care model. Understand and apply relevant legislation and best practices related to older adults (e.g., consent/capacity, substitute decision-making, elder abuse pathways) within scope and organizational policy. Understand and follow Seniors Health Clinic and agency policies and procedures. AdministrativeComplete OT intake, functional and cognitive screening assessments, outcome measures, and required data entry. Create, implement and update client-centred care plans and functional goals in collaboration with clients, caregivers, and the interprofessional team. Complete required documentation with excellence, following agency policy and procedures and respective professional college requirements. Client CareProvide comprehensive occupational therapy assessments for older adults, focusing on function, mobility, cognition, and safety in clinic and community settings. Assess and support clients living with frailty, chronic conditions, pain, mobility limitations, sensory changes, and/or cognitive impairment (e.g., dementia, delirium) as applicable. Support clients to optimize activities of daily living and instrumental activities of daily living (e.g., bathing, dressing, meal prep, home management, community mobility, transportation planning). Deliver falls prevention and safe mobility interventions, including balance strategies, assistive device training, and environmental risk reduction. Complete home safety and environmental assessments (as appropriate) and recommend/coordinate adaptive equipment and minor modifications. Provide cognitive screening (within training and scope) and functional cognitive interventions (routines, memory strategies, compensatory approaches), and support caregivers. Teach energy conservation, pacing, and fatigue management for chronic disease and frailty, supporting return to meaningful activities. Support safe medication routines in collaboration with nursing/pharmacy (e.g., pill organizers, reminders, environmental set-up) within scope. Coordinate referrals and linkages to community resources (e.g., Home Care, rehabilitation services, social supports, vision/hearing services, transportation, seniors benefits). Provide caregiver education and coaching (e.g., safe transfers, dementia communication strategies, environmental set-up, caregiver burnout prevention). Support transitions of care (e.g., post-hospital discharge follow-up, moving to supportive living/long-term care, equipment planning and safety). Identify, document, and escalate safety concerns (e.g., falls risk, wandering risk, self-neglect) following clinic processes and interdisciplinary care planning. Advocate for clients' functional needs and accessible environments, including accommodations, adaptive strategies, and appropriate supports. Contribute to quality improvement, program development, and evaluation activities related to Seniors Health OT services. TeamDemonstrate exceptional written and oral communication skills, including clear documentation and collaborative care planning. Actively participate in daily huddles, case conferences, and coordinated care planning with the Seniors Health Clinic team. Demonstrate initiative and an ability to work as an influential member of a team under minimal supervision, supporting colleagues as needed. Display a high degree of professionalism as well as collaboration with internal programs and external partner agencies to coordinate client care. Provide consultation, coaching, and education to colleagues on function, safety, adaptive equipment, and age-friendly practices. Commitment to contributing positively to team culture. Perform additional duties as assigned.

What You Bring

Degree in Occupational Therapy Active registration and in good standing with the Alberta College of Occupational Therapists. Up-to-date knowledge of geriatric occupational therapy practice, functional assessment, and evidence-based interventions for older adults. Experience working with older adults with complex medical conditions, frailty, mobility limitations, and/or cognitive impairment (e.g., dementia, delirium). Experience working in interprofessional primary care, community health, home care, rehabilitation, or geriatric settings is preferred. Strong knowledge of local/community resources for seniors (e.g., Home Care, supportive living/long-term care pathways, mobility/transportation, financial benefits, caregiver supports). Understanding of age-friendly, culturally safe and trauma-informed practice; awareness of health equity and the social determinants of health. Comfort meeting individually with clients in clinic and, when required, in the community/home settings, with a safety-first mindset. Knowledge of evidence-based OT approaches (e.g., falls prevention, cognitive strategies, pain management, chronic disease self-management, motivational interviewing). Exceptional communication, organization, advocacy, teaching, and rapport-building skills. Commitment to contributing positively to team culture. Openness to a broad scope of practice with a commitment to innovation and flexibility. Ability and willingness to fit/train adaptive equipment and support clients with functional tasks (e.g., safe transfers, mobility aids, home safety recommendations) within scope. Strong initiative in times of limited supervision. Minimum of 21 years of age and three (3) years driving experience is required to drive an Alex fleet vehicle. Class 5 driver's license, personal vehicle for business use, and a clean 3-year drivers abstract. First Aid (Basic or Intermediate) - Level C CPR & AED within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire. A satisfactory vulnerable sector criminal record clearance is required. Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is preferred.

Work Environment and Hours:

Primarily clinic-based in the Seniors Health Clinic: 630-9 Ave SE, Calgary, AB, with occasional home/community visits. Standard hours are 8:00 am - 4:00 pm, Monday to Friday; flexibility may be required to meet urgent clinical and team needs. 1.0 FTE (37.5 hours/week) Continuous extended periods of sitting, standing, walking and occasional climbing of stairs throughout shift. Occasional driving as part of work duties (e.g., home visits, community partner meetings, equipment delivery/pick-up). Occasional time outside in all weather conditions.

Driving on Behalf of The Alex


This position requires the use of a personal vehicle for work-related purposes and driving an Alex fleet vehicle. Successful candidates must show proof of a class 5 driver's license, a personal vehicle, and a clean 3-year drivers abstract. Candidates must consent to the possibility of an annual driver's abstract review to meet our insurance provider's requirements and to maintain $2 million liability insurance on their personal vehicle. Driving a fleet vehicle at The Alex requires that staff are at least 21 years of age with a minimum of three (3) years' driving experience. Mileage will be reimbursed in alignment with the CRA guidelines.
