Detailed Overview
Under the direction of the Director of Care, the Occupational Therapist (OT) is responsible for organizing and implementing rehabilitation programs for residents of St. Michael's Centre (SMC); and to meet the needs of the residents in accordance with the philosophies and objectives of SMC's Therapies Department.
Responsibilities
Organizes and carries out appropriate therapeutic interventions for residents.
Provides required in-service education for staff and residents, within his/her therapeutic scope and professional knowledge, such as lift and transfers, safe use of wheelchairs/power chairs, and best practice(s) related to bed positioning to reduce pressure sores.
Supervises and directs the team of Rehab. Assistants.
Communicates relevant information to the Director of Care and staff in a manner that supports the safe and effective care of residents.
Provides occupational therapy assessment of the patient's physical and cognitive status, functional abilities, needs, and goals by methods such as reviewing records, gathering information, interviewing the patient, identifying patient and therapist expectations, selecting and applying evidence-based assessment methods, consulting with members of the health care team, performing hands-on patient examinations, and monitoring the patient's health to improve, maintain or support occupational health and performance
Analyzes the patient's needs, establishes occupational therapy diagnosis and prognosis and develops an evidence-based, outcome-focused intervention strategy; utilizes assessment and reassessment findings, discusses the analysis with the patient and caregivers, develops short and long term goals and selects a practice model/intervention consistent with patient needs and goals to achieve optimal health outcomes.
Implements interventions in accordance with established treatment guidelines and educates patients and caregivers regarding self-management, health promotion and injury/disability prevention.
Maintains required statistics and records as required to track resident progress.
Participates in facility Resident Care Conferences.
Collaborates with patients and their families/care givers in developing the care plan and encourages family participation in the development, implementation and ongoing modification of the care plan. Establishes a therapeutic relationship by demonstrating empathy, trust and respect, advocating for the patient's rights to privacy, dignity and access to information, and by ensuring the patient's choice and autonomy in decision-making and care planning.
Maintains a variety of records including patient files by gathering information, entering data into computerized databases and/or documenting information into approved formats. Notes patient progress, changes to treatment plan, and records test/treatment results to provide evidence and rationale that support assessment findings and intervention plan. Compiles statistics on workload activities and summarizes information into written reports as requested.
Delegates components of treatment to rehabilitation assistants by providing work direction and verbal and/or written instruction, ensuring and documenting the appropriate transfer of function and ensuring quality patient care.
Develops and maintains working relations with external agencies such as wheelchair suppliers, vendors, etc.
Provides direction to practicum students and volunteers when required
Performs other duties as required.
Qualifications
Graduate from a recognized program in Physiotherapy or Occupational Therapy.
Current registered membership with the Canadian Association of Occupational Therapists, and the BC College of Occupational Therapists or Physiotherapists Association of British Columbia.
Recent related experience of, at least, 2-3 years is preferred. Long Term Care experience preferred
Skills and Abilities
Ability to provide initial and ongoing functional, cognitive and perceptual assessments of residents including transfers, swallowing and wheelchair mobility and seating.
Ability to relate appropriately and effectively to Long Term Care residents and their families, staff and volunteers. Documents assessments on resident chart.
Ability to organize workload independently and work with minimum supervision.
Ability to develop and facilitate ongoing rehabilitation procedures for residents that may be implemented by other staff members such as care plans, equipment provision and training.
Ability to work effectively with other members of the interdisciplinary team and communicate relevant information to assist with ongoing resident care.
Ability to plan and implement rehabilitation programs for long term care residents.
Ability to communicate effectively in English, both written and verbal. .
Effective leadership abilities, including conflict resolution, and the ability to help guide individuals to act wisely and thoughtfully.

Basic knowledge of research process and methodology.
Ability to teach and provide effective consultation to patients, caregivers, rehabilitation assistants and other health care professionals.
Knowledge and understanding of the gerontology, the aging process and Long Term Care is required.
Ability to operate related equipment including relevant software applications.
Physical ability to perform the duties of the position.
