We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapy services to diverse patient populations, including individuals with disabilities. This role involves developing personalized care plans, assisting patients with activities of daily living (ADLs), and supporting recovery and independence across various settings The Occupational Therapist will play a vital role in improving patient outcomes through evidence-based interventions and collaborative case management.
Duties
Assess patients' functional abilities and develop individualized treatment plans aimed at improving independence in daily activities.
Complete In Home Assessments and Form 1.
Implement therapeutic interventions tailored to specific populations such as children with autism, patients in acute care or ICU settings, and residents in skilled nursing facilities.
Work with patients across various age groups in settings like pediatrics, geriatrics, outpatient clinics, inpatient units, and school environments.
Collaborate with multidisciplinary teams to develop comprehensive care plans that address physical, cognitive, and emotional needs.
Provide education and training to patients and their families regarding therapy techniques and adaptive strategies.
Maintain accurate documentation of patient progress and treatment outcomes in compliance with healthcare regulations.
Participate in case management activities to monitor patient progress and adjust care plans accordingly.
Stay current with advances in occupational therapy practices, physiology knowledge, medical terminology, anatomy knowledge, and relevant clinical guidelines.
Skills
Strong knowledge of Form 1 and Attendant care Assessment
Experience working with MVA Patients.
Familiarity with post-acute care environments including home health and outpatient clinics.
Ability to develop effective care plans tailored to individual patient needs across diverse populations such as geriatrics or pediatrics.
Excellent case management skills with the ability to coordinate multidisciplinary services effectively.
Proficiency in medical terminology, physiology, anatomy knowledge, and speech therapy principles relevant to occupational therapy practice.
Ability to work independently as well as part of a collaborative healthcare team in various clinical settings.
