If you are passionate about what you do, motivated to improve the health of the community, committed to excellence, quality and patient safety we would like you to join our Better Together team!Position Summary:Trillium Health Partners (THP) is excited to open an 18 bed Geriatric Unit at our Mississauga Hospital location and is recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and interprofessional skills. As a member of the Geriatric Medicine Interprofessional team you will have the opportunity to provide patient-centred care to the frail older adult with the goal to prevent cognitive and functional decline during admission and participate in plans for same discharge home. This role will report to the Clinical Manager of Seniors\' Services.Key Qualifications:

  • Registered to practice with the College of Occupational Therapists of Ontario
  • Candidate will have relevant experience in working with geriatric patients and a thorough understanding of geriatric syndromes and their impact on function
  • Demonstrates the ability to work within an interdisciplinary team including families and community partners, using effective communication skills
  • Able to work in a fast paced environment within a highly collaborative model of care, putting patients\'needs first
  • GPA training is considered an asset
  • Demonstrates excellent organizational skills and is able to be self-directed in a fast-paced, flexible working environment
  • Previous experience and ability to work effectively and collaboratively with Occupational Therapist Assistants , and other interprofessional team members.
  • A second language is considered an asset
  • A positive attendance record is essential
Responsibilities:
  • The Occupational Therapist is accountable to assume new caseloads comfortably and competently
  • As an active member of an interdisciplinary team is responsible for providing Occupational Therapy assessment and treatment to patients to improve function
  • Provides Occupational Therapy services in keeping with the College guidelines and Essential Competencies, for clients with deficits in areas of occupational performance
  • Participates in Professional Practice meetings and patient care rounds as required
  • Supervises support personnel, students and volunteers as appropriate
  • Participates in program planning, continuous quality improvement and evaluation, thereby contributing to the overall vision/mandate of Trillium Health Partners and Occupational Therapy Services
