Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for an Occupational Therapist to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. This position works within a collaborative multidisciplinary team consisting of various healthcare professionals from the nursing and allied health professions. This team collaborates and conducts home or site visits to deliver client-centered services to individuals with a wide variety of physical, cognitive and mental health conditions, including complex health needs. Using strong assessment and organizational skills, you will identify needs of the client and determine the appropriate internal and external resources required. Through the development of the Care Plan, in collaboration with the client, you will ensure services are provided and modified on an ongoing basis, based on the client\xe2\x80\x99s response to the intervention. This position will be based out of the Strathcona Home Care office but will provide support to home care\'s Suburban/Rural East team, which will include travel to the other home care network offices, as required.

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client\xc2\xbfs physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.
Required Qualifications: Completion of bachelor\'s degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT). Additional Required Qualifications: Bachelor of Science Degree or Master\xe2\x80\x99s Degree in Occupational Therapy. Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Ability to perform all duties in accordance with related legislation and regulatory bodies. Proven ability to provide exceptional clinical care within a continually changing healthcare environment. Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. The successful candidate will work independently in clients\xe2\x80\x99 homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Preferred Qualifications: Previous Continuing Care experience. Palliative care experience. AADL authorizer. Experience with Meditech, Connect Care and the RAI HC assessment tool. Excellent interpersonal and communication skills. Excellent organizational and time management skills.
