The Occupational Therapist is an integral member of the Seniors Outreach and Consult Team. The position provides advanced clinical advice regarding the specialized needs of an identified at risk geriatric population. Uses knowledge to develop, plan and implement treatment and care plans and communicates same to the Seniors Outreach and Consult Team. Ensures staff are coached and mentored on the specific needs of the identified geriatric patients and works closely with community partners and resources to ensure supports are in place for successful transition to the community. Develops education programs and resources for health care staff, patients and their families, and uses a variety of teaching strategies to impart valuable information. Provides leadership and expertise in the evaluation of clinical activities, monitors patient outcomes and adjusts care plans as required. Maintains patient records by charting progress, trends and other relevant information. Participates in relevant research projects, keeps abreast of current research findings and new developments, communicates with the broad team, and incorporates into clinical practice. Baccalaureate Degree in Occupational Therapy. Four (4) years\xe2\x80\x99 recent related experience in an acute and/or outpatient clinical setting, including two (2) years\xe2\x80\x99 experience working with the specialized needs of geriatric patients, plus completion of additional post-graduate courses related to gerontology and treatment of the elderly, or an equivalent combination of education, training and experience.Current registration with the College of Occupational Therapists of B.C. Eligible for membership with the Canadian Association of Occupational Therapists.Skills and Abilities

Ability to operate related equipment.Duties and Responsibilities3.1 Develops, plans and implements advanced occupational therapy assessments to an identified at-risk geriatric population (e.g., complex patients with multiple diagnoses) in a coordinated approach and in accordance with established standards of practice and program standards by methods such as identifying patient issues/needs, planning, implementing, evaluating, and documenting clinical services. Collaborates with other members of the interdisciplinary team, the patient and family to provide advanced advice in the creation of an appropriate ongoing care plan for designated geriatric patients including discharge planning.3.2 Works collaboratively with community partners to ensure supports are in place for geriatric patients returning to patient/family home and arranges supplies and equipment as needed. Develops educational material for geriatric patients and families such as instruction sheets, pamphlets and manuals.3.3 Develops education programs and learning resources for health care team members, and for patients and their families based on industry best practices, regarding the management of the functional effects of the aging process by methods such as researching and reviewing professional literature, consulting with other professionals, and reviewing and modifying education programs.3.4 Utilizes a variety of teaching/learning strategies to support education activities throughout the site(s) by methods such as conducting formal and informal presentations, demonstrations and lectures to a wide variety of health care professionals through coaching, mentoring, and facilitating the desired outcomes. Arranges and provides in-service sessions using educational technology and self-directed learning methods to implement planned programs. Role models direct patient care skills and practices to staff at Providence Health Care and promotes skill development, observes staff while performing patient care, and provides feedback to enhance quality patient care.3.5 Collaborates with other members of the inter-professional team to develop and plan programs, services and resources required by the identified geriatric population. Acts as a clinical resource in meetings including Professional Practice meetings; provides expertise, input and recommendations to the team, with the goal of enhancing patient care, service delivery, and clinical outcomes.3.6 Participates and/or leads continuous quality improvement activities by methods such as evaluating clinical activities and services, selecting and utilizing outcome measures to identify and troubleshoot inconsistencies, evaluate effectiveness of changes in patient care practice, attending and contributing to interdisciplinary meetings, and providing advice as required.3.7 Establishes and builds positive and effective working relationships with a variety of stakeholders such as members of the Seniors Outreach and Consult Team, community partners, hospital staff, the patient and family members.3.8 Coaches and mentors patient and family members on administering treatments, use of equipment and expected outcomes. Acts as an advocate for the patient as needed. Encourages and participates in open communication and acts as a positive role model and mentor.3.9 Provides clinical supervision to and evaluation of students as required.3.10 Provides work direction and oversees designated treatment activities of Rehabilitation Assistants.3.11 Maintains and updates own clinical and professional knowledge of current developments in geriatric occupational therapy by methods such as reviewing professional literature, consulting with other professionals, actively participating in ongoing relevant education programs. Establishes own measurable goals and objectives, and reviews progress to ensure that goals are achieved within established timeframes with the Professional Practice Leader, Occupational Therapy.3.12 Participates and collaborates on research projects, identifies implications of research findings and imbeds new developments into the program, procedures and processes used for the geriatric population identified. Educates members of the interdisciplinary team on most current information relating to occupational therapy interventions, trends and evidence best practices, and updates educational material and/or policies to reflect changes.3.13 Maintains administrative reporting such as completion of workload measurement information, records and statistics as required.3.14 Maintains patient records by methods such as documenting patient assessments, charting patient information, preparing progress notes and reports in accordance with established standards, and policies and procedures to meet regulatory requirements, and documenting for future reference.3.15 Performs other related duties as assigned.
