Crosstown Family Health Team (CFHT), in partnership with the Weston Family Health Organization, delivers comprehensive, team-based primary health care to a diverse rostered population. Our inter-professional team includes physicians, nurse practitioners, nurses, social workers, dietitians, and other allied health professionals, all working collaboratively to support patients across the lifespan with a focus on health promotion, disease prevention, and chronic disease management.
We are currently seeking a dedicated and experienced Nurse Practitioner (NP) to join our team. This is a critical role within our organization, requiring clinical expertise, collaborative spirit, and a commitment to patient-centered care.
Key Responsibilities:
Provide comprehensive health assessments, diagnosis, and treatment planning for patients of all ages
Identify and manage acute and chronic conditions, including psychosocial issues
Collaborate with patients and families to develop individualized care plans
Lead or support targeted programs (e.g. chronic disease management, women's health, child and family health, cancer screening)
Deliver patient education, health promotion, and preventative care
Work closely with the inter-professional team to ensure coordinated care
Participate in case reviews, team meetings, and quality improvement initiatives
Support after-hours clinic coverage as part of regular schedule
Contribute to research, clinical program development, and evaluation
Engage in community outreach and partnerships where needed
Qualifications:
Registered Nurse in the Extended Class (RN-EC) in good standing with the College of Nurses of Ontario
Completion of a recognized Primary Health Care Nurse Practitioner program (Master's preferred)
Preference of a minimum 3-5 years of clinical experience in primary care, community health, or a nurse-led clinic. Training for recent graduates is possible
Proven skills in chronic disease management and health promotion
Demonstrated ability to work collaboratively in a fast-paced, inter-professional environment
Strong communication, organizational, and EMR documentation skills
Cultural competence and experience working with vulnerable or marginalized populations
Experience with electronic medical records (e.g. Oscar based EMR an asset)
