Position Summary:
We are seeking a compassionate, dedicated
Nurse Practitioner
to join our multidisciplinary team on a full-time basis. The Youth Wellness Hub Ontario "YWHO" for the City and District of Thunder Bay will be a youth friendly, culturally safe, low barrier "one stop shop" for youth ages 12-25 to access a wide range of services and support. Core services will include walk-in mental health and substance health counselling, primary care, peer mentorship, and system navigation. Also available will be employment support, sexual health, life skills and recreational activities. Traditional Indigenous ceremonies and land-based activities will be facilitated at the Hub site(s). The successful candidate will provide high-quality integrated youth services to support the well-being of young people at youth hub sites and patient care at our clinic working closely with our Nurse Practitioners and other healthcare providers within the City of Thunder Bay and district.
About the Clinic:
The Lakehead Nurse Practitioner-Led Clinic (LNPLC) is a primary care clinic focused on providing high-quality, patient-centered healthcare in Thunder Bay, Ontario. Our team of Nurse Practitioners, along with other healthcare professionals, work collaboratively to deliver comprehensive care to individuals in the Thunder Bay area.
Key Responsibilities:
Provides comprehensive episodic primary care to individuals' ages 12-25 years of age.
Preform compressive assessment that includes completing health history, collection of information pertaining to the presenting problem.
The NP will collaborate with community partners, family, caregivers with the consent of the patient to collect collateral information that can help to inform the assessment, diagnosis and treatment plan. Identifies and communicates a medical diagnosis to the youth.
Provides care for youth and/or monitors the ongoing therapy of youth. When necessary and appropriate consults with other members of the interdisciplinary team to provideinterventions and comprehensive care, including prescribing drugs, ordering laboratory tests and emphasizes health promotion, education and disease prevention with patient.
Make referrals where necessary to other community providers.
Key Responsibilities Continued:
Employs nursing communication skills in all interactions (e.g., active listening, respectful and culturally appropriate verbal and non-verbal communication, compassion, empathy, etc.).
In collaboration with youth, support the use of YWHO standardized measures to support measurement-based care to monitor progress, and improve youth outcomes.
Participates and/or leads in complex care planning, including assessing, evaluating, amending plans of care after ongoing evaluation of the youths' needs in collaboration with the interprofessional team and our LNPLC team.
Performs selected diagnostic and therapeutic procedures and pharmacological agents and therapeutic measures within scope of practice.
Integrates psychotherapeutic approaches into their clinical visits.
Provides leadership advocating for the clinic's changes to enhance safe patient care.
Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
Provide services and support to youth and their families in a way that is consistent with the values of the YWHO Model.
Provide access to services both in person, remotely (off site) or virtually (online, phone) as needed.
Advocate and facilitate the principles of self-care to youths and team members.
Practice in accordance with College of Nurses of Ontario Practice Standards and LNPLC expectations.
Sensitivity to cultural and socio-economic concerns, and health related matters with individuals facing barriers to accessing care (housing, mental health challenges or diagnoses, substance use, complex health histories).
Education, Qualifications and Skills:
Primary Health Care NP (PHC-NP) Certificate with registration in the Extended Class, with the College of Nurses of Ontario.
Three (3) years' experience in advanced nursing assessment skills in primary care or community health setting is preferred.
Demonstrated interpersonal, communication, problem solving, and leadership skills.
Excellent communication skills and the ability to build strong relationships with patients and the healthcare team.
Experience with electronic medical records system is an asset.
Proficient in use of computers and information technology.
Ability to work independently and as part of a team.
A commitment to ongoing professional development.
Current Ontario Driver's License and access to vehicle
Current BCLS
Up to date on vaccinations (including MMR and Covid) -
must provide immunization record
