The nurse practitioner (NP) is responsible for operating within their full scope of practice, for the advancement of clinical knowledge and professional practice to ensure evidence-based, individualized care of patients and our community. The nurse practitioner will provide direct primary care, and participate in the development, implementation and promotion of health and prevention of disease, while providing clinical direction and care for patients and their families. We are a busy Family Physician office with a Walk in Clinic onsite.
Duties of the NP will be divided into three categories clinical duties, health care team responsibilities and maintenance of the practice environment.
Overview of Role:
Work alongside the MD, provide shared care for patients' health needs, health assessments, health promotion and disease prevention.
Assess, investigate, diagnose and treat patients across the lifespan, utilizing critical thinking and evidence-based medicine and nursing as a guide to practice.
Make referrals independently to physician specialists as per CNO Standard of Practice.
Provide expert coaching, guidance and counselling; patient education, supportive counselling, grief counselling, CBT if qualified.
Provide patient and family-centered care
Maintain accurate, concise, and confidential Electronic Medical Record (EMR) documentation of the patient visits, history, health care needs, test results and consultants' notes. Monitor EMR to recall patients for health promotion and disease prevention follow-up such as immunizations, preventative care, bone density, diabetes care, test results and consultants' notes.
Review and manage incoming prescriptions and renewals with the MD.
Maintain patient confidentiality, privacy and work in a manner that ensures patient safety.
Qualifications:
Minimum masters level nurse practitioner preparation
Current registration in the Extended Class with College of Nurses of Ontario (Successful completion of Ontario NP-Adult Extended Class examination)
Advanced assessment and diagnostic reasoning skills
Strong problem solving and negotiation skills
Must be able to practice independently
Demonstrated leadership to advance the practice of nursing and inter-professional practice to achieve established goals
Excellent communication skills and the ability to develop positive relationships with patients, families, physicians and inter-professional team members
Demonstrated use of theory and research to enable evidence based practice
Current Basic Cardiac Life Support certification
Current Advance Cardiac Life Support certification
Satisfactory performance and attendance required
