Role Overview

The NP will deliver high-quality primary care across three modalities:

In-office visits (scheduled assessments, chronic disease management)
Virtual visits (follow-ups, symptom management, care plan reviews)
Home Visits (scheduled and emergency visits to frail/ complex seniors)
Team support (clinical guidance for LPNs and RNs, on-call rotation)

In addition to clinical work, the NP will report directly to and collaborate closely with the Medical Director to help design the Monarch Health Playbook -- a standardized set of protocols, workflows, and clinical guidelines that will support practice scaling and ensure consistent, high-quality care.

Key Responsibilities

-Clinical Care
-Provide comprehensive in-office and virtual assessments
-Diagnose and manage acute and chronic conditions
-Update care plans, review labs, and communicate results
-Coordinate care with specialists and community providers
-Maintain high-quality EMR documentation
-Team & Operational Support
-Offer real-time clinical support to LPNs and RNs
-Participate in a shared phone-based on-call rotation
-Contribute to case reviews, huddles, and clinical discussions
-Practice Development (Playbook & Protocols)

Work with the Medical Director to:

-Draft clinical protocols and decision-support tools
-Develop workflows for patient intake, triage, follow-up, and escalation
-Standardize care pathways for chronic disease management
-Help create templates, cheat-sheets, and care algorithms
-Support quality improvement and operational optimization
-Participate in iterative testing and refinement of protocols
-Document workflows for training, orientation, and scalability

Qualifications:

Active Nurse Practitioner licence (BC)
Primary Care NP experience preferred
Experience with chronic disease management, complex care, home-based care, or geriatrics is an asset
Strong clinical judgment and comfort practicing independently
Excellent written and verbal communication
Experience drafting or following clinical protocols is an advantage
Valid BC driver's licence (if occasional field support is needed)
