Nurse Practitioner - Primary Health Care ( Clinic)

To support this, we are embarking on an exciting new journey to ensure that patients in our Tottenham town have equitable access to safe, effective, and efficient healthcare services. With the launch of a remote/virtual walk-in clinic. We are looking for an experienced professional to fill this key role of Primary Healthcare Nurse Practitioner.

Assess and evaluate acute and chronic health conditions for patients with an emphasis on assessment, diagnosis, treatment, follow-up, counselling, screening, education and referral. Provide health education and counselling focused on health promotion and disease prevention. Improving patient outcomes along the continuum of care through utilization and application of up to date evidence based guidelines, interdisciplinary collaboration, and participation in research. Provide leadership to promote program development, process improvements and the enhancement of professional standards within the clinic or virtual care platform. Work collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of care. Also to Diagnose foot conditions ( foot care provided by podologist) Additional duties needed as per the scope of the NP practice and needs of the patient population and organization.

We are seeking experienced Nurse Practitioners who is passionate about delivering meaningful frontline, clinic-based collaborative primary care in Tottenham community. Successful candidates will work independently within their full scope of practice to provide in-clinic and virtual patient care.

As a Nurse Practitioner, you will play a pivotal role in providing patients with comprehensive primary healthcare services. Your role will include conducting patient assessments, diagnosing and treating acute and chronic conditions, and developing personalized care plans with a focus on health promotion.

Job Type: Part-time

Pay: $50.00-$70.00 per hour

Expected hours: 6 - 8 per week
