Job Description

Mackenzie Health LTC, located in Richmond Hill, Ontario is a 170-bed long-term care home dedicated to enriching the lives of our residents in partnership with UniversalCare, our family members, valued employees and community partners.

Our team is currently hiring for a permanent full-time Nurse Practitioner. Reporting to the Administrator, the Nurse Practitioner position is a critical role with overall accountability for the management and leadership for health care and services for our residents.

As part of our team, your role as a Nurse Practitioner contributes to our resident experiences in the following ways:



In collaboration with the leadership team and other interdisciplinary team members ensures the provision of care is aligned to the needs of each resident, the home's strategic plan, mission and values. Promotes a resident-centered care approach that engages residents, family members and employees to provide care that meets the individual needs of our residents. Completes daily clinical rounds, orders, interprets and evaluates diagnostic tests for residents to identity and assess resident's clinical concerns and health care needs. Records physical findings, formulates action plans in collaboration with the other interdisciplinary team members and prescribes or recommends drugs or other forms of treatment to improve resident care. Provides leadership and direction in collaboration with internal and external stakeholders for all activities related to nursing, evaluates clinical outcomes and works with other interdisciplinary team members to continuously improve these outcomes. Ensures compliance with all relevant legislation including but not limited to: Ministry Acts, regulations, policies and procedures, directives, and collective agreements. Collaborates with the Medical Director, other physicians and community services/agencies in the provision of quality health care for the resident. Conducts physical examinations and preventative health measures within prescribed guidelines and the standards of the provincial College of Nurses. Provides assessments for residents with High Intensity Needs (HIN). In collaboration with the Director of Care, provides information for Critical Incidents, as needed. Assist with onboarding of new hires and education of staff, as required. Mentors and develops nursing staff with respect to nursing practices. Actively participates in conferences, workshops and other educational opportunities for further development of best practices and nursing knowledge. Works in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act. Performs other duties as assigned.

Qualifications:



Current registration as a Nurse Practitioner, in good standing with the provincial College of Nurses.
Must have a minimum of one (1) year's working experience as a Nurse Practitioner in a healthcare setting, preferably long-term care.
Strong relationship building abilities and organizing skills with the ability to lead and influence others.
Must posses excellent written and oral communication skills.
Demonstrates knowledge of current trends and best practices relative to nursing.
Sound knowledge of the Fixing Long-Term Care Homes Act 2021, Ontario Regulations 246/10.
Proficiency with Microsoft Office, Point Click Care and other computerized documentation systems.
Must possess current CPR certification.
Certificate in Gerontology and current membership with the GNA would be considered an asset.
All applicants must successfully pass the prescribed Vulnerable Sector check.
