Nurse Practitioner
Job Description
Prescott Family Health Team is seeking an experienced Nurse Practitioner to join our team. As a member of our team, you will provide ongoing comprehensive primary care to clients of all ages within the Primary Care Nurse Practitioner scope of practice., including assessment, diagnosis, treatment and follow up; counseling, referral and client education. Also, but not limited to below;
See patients both on a regular scheduled visit and/or urgent basis.
Perform clinical assessment and complete accurate patient records, assessments notes and required correspondence.
Assist with our preventative care and patient education programs.
Assure confidentiality and privacy of client information in all processes.
Follow all infection control policies and procedures with all processes and procedures.
Current registration and in good standing with the College of Nurses of Ontario is required.
