Assets & Specialties:

Comprehensive Patient Assessment, Diagnosis and Management of Acute and Chronic Conditions, Prescribing Medications (within scope), Diagnostic Test Ordering and Interpretation, Health Promotion and Disease Prevention, Patient and Family Education, Care Coordination and Case Management, Wound Care Management, Mental Health Assessment and Support, Experience in Multidisciplinary Team Settings, Strong Communication and Collaborative Skills, Comfort with Independent Practice, Commitment to Ongoing Professional Development to name a few.

Requirements:

Active registration and in good standing with the College of Nurses of Ontario. Professional and Commercial general liability insurance. An existing client base or the motivation to build one. A desire to engage in a collaborative, wellness-focused environment. An updated CV. 2-3 references.
