Cedarwood Village Long Term Care is looking for a dynamic Nurse Practitioner to join our health care team in Simcoe, Ontario.

We are seeking a self-directed, results oriented nursing leader who demonstrates strong communication skills, critical thinking and recognizes the importance of relationship building with all team members.

You will provide innovative and creative clinical leadership for our inter-professional team to build capacity and support knowledge development in geriatric care; and work collaboratively with our physicians, staff and system partners. Experience in Palliative and End-of-Life care an asset.

RESPONSIBILITIES



70% of the NP's time must be engaged in direct care to residents, 30% to be engaged in research, education and leadership Provide primary care within the scope of practice of the Nurse Practitioner as outlined in the College of Nurses of Ontario Standards of Practice for Registered Nurses in Extended Class. The provision of primary care includes assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up Collaborate effectively with physicians to facilitate appropriate provision of primary care outside of scope of practice Liaise with physicians and community agencies as required Maintain complete, accurate and timely records of patient visits using the electronic medical record (EMR) Communicates finding and diagnoses, discusses health outcomes to resident, families and the care team Provide individual and group sessions to patients in the understanding and management of particular health issues Prepare statistical reports for program review when necessary Work effectively within an interdisciplinary team to improve patient access to primary care and to help improve the team's ability to provide a broader range of services (e.g. wound care management) Evaluate and analyze the patient's response to therapeutic interventions Participates in internal (e.g., Professional Advisory Committee) and external committees Provides leadership and involves the inter-disciplinary team in quality improvement initiatives Provides leadership in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities Provides formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as a resource person, educator and role model

QUALIFICATIONS



Nurse Practitioner Certification from a recognized program Must possess current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC) and be in good standing Must maintain competencies as per the governing College standards Membership with Nurse Practitioners' Association of Ontario (NPAO) Professional practice liability insurance Demonstrated competency in advanced assessments, diagnosis and prescribing pharmaceuticals and non-pharmacological interventions within the legislated scope of practice Must have demonstrated sound clinical knowledge and ability to delegate to unregulated care providers Clear understanding of Resident-Centered Care

Superior interpersonal, communication, organization and team skills Excellent oral/written skills Strong clinical skills Ability to work effectively with all members of the health care team Proficiency in the use of computer hardware and software Ability to prioritize, manage time effectively and be flexible in a very active work environment Willingness to teach and train other members of the interdisciplinary team as required Innovative, creative, flexible and enthusiastic Flexibility to take on other tasks as assigned
