JOB SUMMARY



We are currently looking for a full-time Nurse Practitioner (NP) to join our team and provide a full range of healthcare services to patients within the primary health care NP's scope of practice. The NP will provide care to patients within our FHT guidelines for health promotion, disease prevention and chronic disease management. The NP will work collaboratively with physicians and allied health professionals in our shared care model.

As a member of our team, you are responsible for bringing issues that affect the smooth operation of the organization, to the attention of your immediate supervisor. It is your responsibility to be up to date with all organizational communication.



ESSENTIAL RESPONSIBILITIES



Initiate and manage the care of patients with diseases or disorders within the Nurse Practitioner's scope of practice and/or monitors the ongoing therapy of patients by providing effective pharmacological, complementary or counseling interventions.



Evaluate effectiveness of care plans by assessing patient comprehension and compliance to the care plan, reviewing clinical, behavioral and learning outcomes.



Based on evidence and best practice and in networking collaborate in development of goals and objectives, services and activities, outcome measures and evaluation for student community. By doing so, develops an information network for present & future use.



Integrate best practice guidelines and current evidence-based research into clinical practice,



Evaluate clinical outcomes and integrate nursing knowledge and research into clinical practice,



Other activities may be assigned in collaboration with the health services members that relate to a Nurse Practitioner scope of practice.



Develop linkages with external partners as appropriate to ensure continuity of care,



Work in a manner to ensure safety to oneself, patients and their families, colleagues and the work environment.



ESSENTIAL QUALIFICATIONS



Current Registered Nurse Extended Class certificate and registration with the College of Nurses of Ontario



Graduate of a recognized Nurse Practitioner program



Current professional liability insurance



Minimum of two years clinical nursing practice experience as a Nurse Practitioner



Previous RN experience in hospital emergency department, primary care clinic or equivalent.



Bilingual preferred (French & English)



Excellent communication and interpersonal skills



Strong clinical and effective time management skills



Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team



Excellent health assessment skills, clinical judgment, analytical and problem-solving skills



Has completed an approved controlled substance prescription and management education program
