Position Summary





The Nurse Practitioner is a member of an integrated health care team that provides service to equity seeking individuals as well as the broader community, emphasizing holistic care, health promotion and disease prevention, and providing care through a trauma and violence informed lens. As a Nurse Practitioner with our Team, you will work both independently and in collaboration with community Physicians and other health care professionals across multiple clinic sites to assess, diagnose and treat health problems.



The Nurse Practitioner, in partnership with other team members and collaborators, seeks to provide culturally appropriate care to all clients as part of their commitment to providing safe, person-centered care. The position requires culturally appropriate care delivery for Indigenous community members.




Key Responsibilities




Client Care




Works within the scope of practice set for a Nurse Practitioner to provide primary care to both patients rostered to community Physicians and community members who are without a primary care provider. Works to design, implement and support specialty clinics (including unattached, harm reduction, vaccination, screening, palliative) in collaboration with the team and community Physicians. Takes into account environmental, social, psychological and medical risk factors (social determinants of health) in a manner that is culturally safe, can be understood by clients, appropriate to their family situation and attuned to their cultural background. Works with and refers to other members of the care team or other health care professionals to ensure clients receive comprehensive, competent and appropriate health care. Acts as a health advocate for clients and uses health care resources effectively. Assesses clients for routine health and illness care, prescribes treatment, prescribes medications, orders diagnostic tests, and promotes chronic disease self-management principles as appropriate. Remains accessible to clients during the hours of service scheduled by KCHC, at on and off-site locations and provides home visits as required. Counsels and educates individuals on health and lifestyle issues and evaluates the effectiveness of education and counseling approaches and strategies.



Administration




Maintains accurate records of clients' visits and encounters, including phone encounters, using the charting and recording formats established within the EMR identified by KCHC. Participates in team meetings and collaborative efforts to improve quality care to clients. Participates in developing, implementing, facilitating and evaluating mental and physical health promotion programs and other community initiatives, including service provision for unattached patients. Identifies areas where development of protocols and procedures are needed to improve client services or to promote more effective staff functioning. Assists in health services planning and programming by participating on committees, supporting programs, and sharing information about identified health needs within the community as obtained through client encounters. Assists other nursing staff in administrative and clinical duties as required, including supporting coverage for other primary care Providers.

Basic Education and Experience Requirements




On-going registration as Registered Nurse (Extended Class), Ontario College of Nurses. Three to five years nursing experience in a primary care setting; or a combination of community, hospital or public health settings. Applicants with less experience may be considered. Experience supporting marginalized clients, such as Indigenous populations, clients with addictions, mental health, and/or complex psycho-social issues.



Knowledge, Skills and Abilities



This position requires culturally appropriate skills and abilities to work effectively with the Indigenous population. Knowledge of management of clients with addictions and mental health diagnosis. Familiarity with trauma informed care. Excellent oral and written communication skills. Excellent interpersonal skills with ability to engage marginalized clients, and a commitment to being a team player. Demonstrated ability to attend work on a regular basis and ability to meet the physical demands of the position. Excellent organizational skills and above average attention to detail. Proficiency in the use of computers and relevant software applications (Microsoft Office), including Electronic Medical Record software (Telus PSS). Cultural safety training for Indigenous clients is an asset. Experience living or working in the Rural Frontenac, Lennox and Addington and Deseronto region is an asset. Experience with Quality Improvement an asset.



Other Requirements




Current and satisfactory Criminal and Vulnerable Persons Check Reliable access to a vehicle and ability to travel between various primary care clinics, the communities of Rural Frontenac, Lennox and Addington, and Deseronto Valid Driver's Licence, Driver's Abstract and proof of vehicle liability insurance as this is a requirement under KCHC's insurance provider (HIROC) Up to date vaccinations
