Join Canada's leading academic transplant center and deliver advanced patient care for lung transplant candidates and recipients. As a Nurse Practitioner (NP) embedded in Transplant Services, you'll practice at full scope within a high acuity, interprofessional team, co-managing complex transplant patients, standardizing pathways across programs, and advancing quality outcomes for transplant patients. Working primarily on inpatient units (with consultative support to ICU/ED as needed), the NP provides comprehensive assessment, diagnosis, treatment and coordination of care for pre- and post-lung transplant patients, using your advanced assessment skills and clinical knowledge. The role partners closely with lung transplant physicians, transplant care nurse coordinators, thoracic surgery, pharmacy, infectious diseases, allied health and program leadership to ensure safe, efficient care transitions from admission through discharge and handover to outpatient follow-up. As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems.
