Are you a Nurse Practitioner looking for a dynamic and meaningful opportunity to make a difference in the lives of older adults? As the most responsible health practitioner (MRPH), this role offers the chance to provide direct client care to complex clients who are homebound, medically frail, and experiencing significant barriers to accessing primary care in traditional environments. You will work in close collaboration with Seniors Home-Based Primary Care (SHBPC) physicians, who serve as the Most Responsible Practitioner, ensuring coordinated and comprehensive care. In addition to supporting SHBPC clients, you will also provide direct client care to clients in the In-Home Restorative Care program (IHRCP). Clients in the IHRCP have recently experienced a decline in their functional ability. Whether due to a health setback or following a medical event, with the goal of supporting clients to regain their strength, mobility, and functional independence. You will work in close collaboration with the client's primary care physician. The position is embedded within a blended interdisciplinary and transdisciplinary model of care, offering a rich environment for collaboration and innovation. Description:
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems.
