The Nurse Practitioner (NP) provides direct patient care to clients of our Agency within bed-based Withdrawal Management and Treatment Services (WMTS). At WMTS you will work within an interdisciplinary team to provide comprehensive biopsychosocial support for individuals with addictions and concurrent disorders. This includes clients in acute withdrawal from one or more substances and those coming into our 5-week bed-based treatment program. Your focus will be on comprehensive health assessment, individualized treatment plans, and supporting continuity of care through collaboration and referrals. The NP at WMTS provides episodic primary care to clients in program and assists with transfer of care to more permanent services. Our NP provides expertise to inform policy decisions where appropriate. The NP will also provide guidance and coaching to other members of the WMTS team, including non-medical staff, to ensure best service possible. The WMTS Nurse Practitioner
Adheres to CPSO prescribing guidelines and CNO Standards of Practice, and demonstrates practice competencies
Conducts advanced mental health/addictions/concurrent disorders and physical health assessment and formulates evidence-informed plan of care
Provides care that is recovery oriented, trauma-informed, and reflects harm reduction principles
Prescribes and administers medication (including OAT) to support safe and comfortable withdrawal and monitors the impact of prescribed medications on clients in withdrawal (impacts on clients' mood, behaviour, cognitive processes)
Conducts medical intake for bed-based treatment, ensuring client stability for duration of the program
Supports the WMTS interdisciplinary team to implement plans of care, including transition plans, to best serve clients
Conducts risk assessments for suicide, self/harm or harm to others and takes the necessary action
Supports education, collaboration, and knowledge exchange to enhance partner, primary care, and community capacity to effectively address addiction
Collaborates with interdisciplinary support team and external partners (i.e., pharmacy, RAAM medical team, hospital) to ensure safe and effective practice and continuity of care
Maintains clinical competence through relevant evidence-based research and legislation
