Job Description

The NP works primarily with the Respirology/CF team to provide advanced nursing care to patients with Cystic Fibrosis as well as working collaboratively with the Respirologists in general Respirology clinics if needed. Reporting to both the Program Director and the Director of Nursing Practice and Education, the NP also works with the Interprofessional team and other Nurse Leaders to support the implementation of key initiatives and programs (including the implementation of best practices) relevant to this patient population.


The NP works within 4 domains of practice:


Clinical Domain:

Working both collaboratively and autonomously with Respirology/CF Team, the NP uses advanced knowledge and skills to:


Provide clinical care to patients and their families as the central part of the NP role. The NP works autonomously to provide comprehensive assessments, make diagnoses, order and interpret relevant diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice and a defined Unity Health Toronto Practice Agreement. Lead the interprofessional team to provide clinical care for patients referred to the Cystic Fibrosis team and engage community care providers as needed to provide holistic care to patients.
Follow in- and out-patients with CF with a focus on assessing and providing mental health supports, assessing at-risk patients (e.g. patients in transition from pediatric programs), prescribing and adjusting medications specific to CF and mental health (in collaboration with Psychiatry as needed).

Knowledge Translation:




Analyze, appraise and synthesize research and best practices to use as a foundation for enhancing the delivery of quality patient care. Use knowledge translation techniques in innovative ways to put research-based knowledge into daily practice and to support and educate other members of the health care team. Contribute to the dissemination of nursing knowledge through presentations and publications.

Scholarship:



Contribute to the culture of discovery at St. Michael's Hospital by advancing and/or appraising nursing research. Evaluate outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends. Identify issues related to clinical practice and patient care and develop innovations to improve care and care delivery for patients, families, and communities. Design, direct, participate in and evaluate quality improvement methodologies to promote safe, timely, effective, efficient, equitable, and patient-centered care.

Leadership:




Act as a transformational leader by using ideals, inspiration and intellectual stimulation to influence the behaviors and attitudes of others towards the goal of excellent patient care Act as a consultant and resource to members of health care team and external groups in the process of decision-making, strategic planning and problem solving by committee involvement, participation in program development and evaluation Act as a mentor to a variety of learners, participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate and lead program development, presentations, evaluations, research initiatives and other system changes to continue the enhancement of the inter-professional care team and program.


Qualifications:




Adult or Primary Health Care NP (Registered Nurse in the Extended Class) in good standing with the College of Nurses of Ontario (or eligible for registration) required Master's degree in Nursing with specialized clinical focus in Nurse Practitioner Practice required Current BCLS certificate required Completion of education to prescribe controlled substances required by the College of Nurses of Ontario preferred Completion of the Certificate in Hospital & Acute Care for Nurse Practitioners (in whole or in part) preferred
Demonstrated experience and leadership in education and research preferred
Current experience and demonstrated success as a Nurse Practitioner and/or Practice in Respirology preferred. Strong understanding of the Cystic Fibrosis patient conditions preferred Experience in mental health preferred Membership in the Nurse Practitioners Association of Ontario is an asset Ability to function as an independent practitioner and a collaborative team member Excellent team-building, negotiation and conflict resolution skills Experience in developing, implementing and evaluating nursing practice tools in an ambulatory setting and primary care framework preferred
Expertise in developing strategies to build capacity and improve chronic disease self-management outcomes preferred Experience and ability to lead and facilitate the development and implementation of basic and advanced education programs for patients, families, staff, and other health professionals preferred
Demonstrated skills and experience in program development and evaluation Demonstrated ability for independent action, initiative, problem solving, critical thinking, organization and prioritization Demonstrated mastery of effective communication with patients and coworkers, both written and verbal Demonstrated ability to work in a consultative role with the members of the inter-professional team Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations Demonstrated ability to provide culturally-sensitive services Demonstrated ability to utilize information technology Committed to acquire new knowledge and ensure competency Eligible for cross appointment with the University of Toronto, Bloomberg Faculty of Nursing
