Job Description

Nurse Navigator - Full-time, permanent



A Nurse Navigator within the Algonquin Family Health Team (AFHT) will aim to coordinate patient care, connecting complex patients with resources, and facilitating communication between patients and the interprofessional professionals with the AFHT and beyond.

This role involves accurately and comprehensively assessing patient needs, navigating the health care system, and providing personalized support to ensure high-quality, patient-centered care across various settings. This role will bridge gaps in care, especially for complex cases, and support patients in achieving their health goals and the mission, vision and values of the AFHT.

PRIMARY RESPONSIBILITIES & DUTIES:



Patient Experience and Outcome Improvement



Monitor and support the patient's health care journey. Communicate with patients, caregiver's families and members of the interprofessional team to facilitate decision-making regarding care. Interact with patients to address questions or concerns.

Coordination and Collaboration



Facilitate and monitor the coordination and mobilization of resources to meet the needs of patients and their families. Collaborate with the AFHT interprofessional team, including referrals and consultations.

Education and Program Development



Assist patients, caregivers and families in understanding their health care options. Participate in the development of education materials for patients and families in the community. Participate in interprofessional education opportunities to increase knowledge and promote collaboration. Support ongoing program development, analyzing opportunities for improvements in the AFHT.

QUALIFICATIONS:



Registered Nurse (Current member in good standing with a College of Nurses of Ontario (CNO)). Certification in Patient Navigation is an asset. Enjoy working with patients who have complex challenges to support and guide them in their primary care journey. Knowledge of community and regional health care resources available to patients and families. Excellent interpersonal, communication, presentation and documentation skills. Skill in organizing resources, analyzing and solving problems, establishing priorities and making evaluative judgements. Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care.
