Within the context of a client and family centered model and in accordance with the College of Registered Nurses of Saskatchewan (CRNS) and Registered Psychiatric Nurses Association of Saskatchewan (RPNAS) standards of professional practice, Code of Ethics and the established vision and values of Saskatchewan Health Authority (SHA), the Palliative Care Coordinator (PCC) provides assessment and access to a comprehensive range of high quality End-Of-Life-Care for patients admitted to palliative care services in any care setting and their family/supporters. Under the direction of the program area manager, the Palliative Care Coordinator designs, implements and evaluates palliative care services provided to clients in the Saskatchewan Health Authority

3 yrs experience in Palliative Care (combo of acute and community preferred)
Education
Bachelor of Science in Nursing (BScN)
Licenses
Valid Class 5 driver's license
Other Education and Training
Bachelor of Psychiatric Nursing is acceptable
LEAP Certification (Learning Essential Approaches to Palliative and End of Life)
Able to plan/organize/prioritize work w/in a continuously changing environment
Ability to build and maintain effective relationships/partnerships
Ability to competently use computers and software applications
Commitment to continued learning and professional development.
Articulates theory and knowledge related to client centered care
Possess a high degree of emotional maturity to interact w/ patients and families
Ability to work independently
Ability to provide clinical recommendations to HCP, clients and their caregiver
