Brief Overview:


The Metis Nation-Saskatchewan (MN-S) is the government that represents Metis citizens in Saskatchewan, and MN-S Ministry of Mental Health and Addictions is responsible for improving the health and well-being of Metis citizens in Saskatchewan. The Metis Reintegration Support Worker will work in an integrated and collaborative way to improve treatment and support access for Metis individuals transitioning out of federal institutions subject to Section 84 of the Corrections and Conditional Release Act (S.C. 1992, c.20). The Metis Reintegration Support Worker will be responsible for conduction of initial assessments, coordinating and monitoring care, advocating, connecting clients with community resources, and identifying addiction, mental health needs, harm reduction, advocating, mentoring, and working with the other ministries of MN-S to support the reintegration of Metis individuals transitioning out of federal institutions.

Key Responsibilities:

Support Metis individuals transitioning from federal institutions subject to Section 84 of the Corrections and Conditional Release Act (S.C. 1992, c.20) Conducting initial assessments to identify clients' mental health and/or substance abuse/use needs and develop individualized care plans which may include counselling, parenting support, educational support, advocacy, and job readiness. Complete safety plans aligning with Section 84 of the Corrections and Conditional Release Act (S.C. 1992, c.20). Monitoring clients care with other healthcare professionals, including psychiatrists, therapists, and primary care providers. Advocating for clients to ensure their needs are met and their rights are protected. Monitoring and recording clients' progress and adjusting care plans as necessary subject to Section 84 of the Corrections and Conditional Release Act (S.C. 1992, c.20). Connecting clients with community resources, such as support groups and social services. Maintaining accurate and up-to-date clients' records. Other duties as assigned

Knowledge and Skills:

Sound knowledge in the Metis determinants of health. Implementing cultural and recreational programming and working in conjunction with the correction liaison workers to identify areas of need for the clients, victims, family and community. Knowledge and understanding of the Federal and Provincial Justice Systems. Knowledge of Mental Health and Addiction related treatment, policies, procedures, and trends. Experience working with Metis people and must be familiar with Metis culture, traditions, values, heritage and language. Ability to work with interdisciplinary teams. Research and evidence-based practices. Operate computer systems and must have knowledge of Microsoft Office and data entry. Excellent interpersonal, communications and organizational skills. Can manage multiple tasks and work with a team or with minimal supervision.

Qualifications:

Bachelor's degree in social work, Nursing, Psychology, or related field Minimum 3 years' experience providing case management services to individuals with mental health and addiction issues. Familiarity with electronic medical records (ERM) systems
