Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on-site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Today, we are active in over 29 retirement residences across Ontario, Saskatchewan, Alberta, and Manitoba, with continued plans for growth and expansion.

Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About The Role



We are seeking a Manager, Clinic Operations to oversee Perennial's operations across Ontario, Manitoba, Alberta and Saskatchewan, with a start date between December 2025 and January 2026. This hybrid position will be based in Toronto, Ontario with a hybrid work model of 3 days in office, 2 days virtual.

The Manager, Clinic Operations is responsible for ensuring top-tier operational performance across all Perennial clinics. This role ensures smooth scheduling, efficient workflows, and strong collaboration with each retirement home operator. A key mandate is driving utilization, patient satisfaction, and scalable processes that support Perennial's rapid growth.

ResponsibilitiesOperational Leadership



Own and oversee all operations across assigned provinces, ensuring smooth workflows at every clinic and residence. Develop and manage provider scheduling, workflows, supply standards, and clinic readiness. Oversee and manage Regional Operations Coordinators and Patient Care Coordinators, providing leadership, training, and accountability. Monitor site-level financials, staffing, and performance metrics, reporting regularly to leadership. Escalate critical risks, issues, or strategic opportunities directly to leadership. Implement standardized operational processes across all locations to ensure consistency and quality.

Client & Partner Management



Maintain strong relationships with Executive Directors, Directors of Care, Marketing Directors, and other home leadership. Serve as the primary escalation point for operational issues at any home. Collaborate with Business Operations and Executive leadership prior to launches and renewals. Drive revenue through allied health partners by actively promoting services and clinic utilization.

Allied-Health Program Execution



Ensure smooth onboarding, scheduling, and operational success of allied-health providers on-site (physio, audiology, chiropody, etc.). Maintain room assignments, equipment coordination, clinic logistics, and performance reporting. Monitor service delivery and resident satisfaction; provide feedback to Business Ops for renewal and contract decisions.

Marketing, Utilization & Engagement



Lead collaboration with homes on monthly marketing activations and stakeholder engagement. Support promotional campaigns, signage deployment, family communications, and resident education events. Track utilization metrics; execute targeted initiatives to increase booking volume and clinic adoption. Promote and organize allied health clinic days, health events, and resident engagement initiatives. Identify and develop new partnerships to expand Perennial's reach and service offering.

Clinical Coordination



Partner closely with the Nurse Practitioner Lead to ensure adequate NP coverage and scheduling across all sites. Support onboarding of new clinical providers and allied health professionals in the region. Ensure compliance with clinical policies, standards, and performance expectations.

Performance Measurement & Reporting



Maintain dashboards and operational KPIs (clinic hours, scheduling gaps, utilization, allied-health ROI, etc.). Conduct regular review meetings with leadership to highlight operational risks and improvement plans. Support budgeting and forecasting for clinic staffing and operational needs.

Qualifications



3-5+ years of multi-site operations experience -- ideally in healthcare, startup, or a regulated service environment. Strong leadership experience managing coordinators and front-line operational teams. Ability to build strong partner relationships and manage escalations diplomatically. Highly organized and analytical with strong operational judgment. Proficient with scheduling systems, EMRs, spreadsheets, and workflow software. Valid driver's license and ability to travel regionally between homes.
