Summary Description



Reporting to the Director of Care, the

Licensed Practical Nurse (LPN)

is responsible for providing personal assistance, supporting activities of daily living and providing comfort and support services to residents under their care. The LPN provides coverage of the Health Centre, provides emergency care and first aid treatment to residents and staff, administers medication and/or other services under the Community's Supportive Care Program. The LPN is responsible for managing the provision of safe effective care that is in accordance with contractual and legislative requirements, and Origin's policies and procedures. The LPN is also responsible for leading, supporting and mentoring staff and health care aids to provide quality care as well as assisting with quality assurance ad reporting. The LPN is responsible for communicating between the nursing staff (LPNs and HCAs) and the RNs, CCCs, Educators, Director of Care and other support staff involved in the resident's care. Lead LPN will be responsible for emergency care and first aid, staffing, mentoring/educating new and support staff, managing safe effective resident care and assisting with quality assurance and reporting.

Education/Minimum Requirements



Any combination of education and experience providing the skill and knowledge for successful performance would qualify. Typical qualifications would be equivalent to:

Practical Nurse Diploma or equivalent accredited nursing education program. Current practice permit or eligible registration with the College of Licensed Practical Nurses of Alberta (CLPNA); permit in good standing. Criminal and Vulnerable Persons police check; in good standing. Current BLS-HCP and First Aid Certification; must remain current. Strong written and oral communication skills or ability to read, write and communicate in English. Experience in the senior health care industry and complete understanding of the aging process.

Skills/Knowledge



Current BLS - HCP and First Aid certification. Experience working with seniors in the community health services setting. Basic computer skills and knowledge of Microsoft Office. Able to multitask, prioritize, and manage time efficiently.
Ability to effectively work independently and within a team.

Physical Requirements



Ability to meet the physical requirements including:
o Lifting to maximum 50 lbs

o Carrying/transporting objects up to maximum 25 lbs

o Bending, crouching, kneeling, reaching, stretching

o Climbing stairs and sometimes working in confined spaces

o Pushing and pulling up to maximum 100 lbs

o Standing for prolonged periods of time

o Sitting at a computer typing for long periods of time

o Frequent walking covering long distances

Duties and Responsibilities

Promotes a safe and respectful workplace, role model for staff.

Lead for scheduling and finding coverage for floor during after hours

Collaborate with RN Educators to ensure mandatory education requirements are met as per standards. Assists in the training and orientation for new LPN and HCA teams.

Develop a working knowledge of provincial regulations, policies and procedures dictated for residents and ensure compliance.

Upholds resident and family centered care and maintains resident confidentiality.

Respond to medical emergencies in accordance with organizational policies and procedures.

Respond to resident/family concerns and ensure appropriate action taken within decision making authority and/or concerns are communicated to supervisor. Collaborate with Site RNs, CCCs, DOC to attempt to resolve issues concerning resident care.

Assist in maintaining a safe a cleanly environment, ensuring facility and resident equipment are maintained.

Observe and report to the supervisor any concerns regarding resident safety or staff safety, including physical environment, and implement interventions as directed by supervisor.

Attending staff meetings as required and attending in-services to maintain and develop knowledge and skills.

Organizes daily flow sheets and provide supervision and direction for care routines for HCAs and LPNs.

Manage and monitor floor coverage, receive absence calls, and efficiently schedule.

Complete wound care management care plans as required. Ensure dressing supplies for resident wound care kits are assembled and maintained as per AHS standards.

Supervises medication administration program and completes medication reconciliations as needed and monthly e-mar reviews.

Review and update care plans on a regular basis.

Participate and promote quality improvement initiatives with RN Educators.

Assist with new employees and preceptorship orientation.

Must meet Alberta Health & Wellness Continuing Health Services Standards.

Admission and transfers of residents.

Performs nursing procedures such as catheterizations, applying and changing sterile dressings and/or non-sterile dressings, colostomy care, taking and recording vital signs (including blood pressure, temperature, pulse and/or respirations), blood glucose testing, administering routine and PRN medications (including performing sub-cutaneous and intramuscular injections), suppositories and/or enemas.

Respond to resident/family concerns and ensure appropriate action taken within decision making authority and/or concerns are communicated to supervisor. Collaborate with Site RNs, CCCs, DOC to attempt to resolve issues concerning resident care.

Records and charts: medication administration, resident observations and assessments, objective and subjective symptoms, problems, changes, and/or concerns and reports to the Director of Care as needed.

Consults with Care Team and provide information to formulate, implement and evaluate nursing care plans for residents.

Provides personal care to residents and assists residents with meals, transports residents within the facility, accompanies residents during outings and social activities, and provides assistance as required.

Works cooperatively with other staff and departments to ensure resident care needs, including dietary and activity program needs, are met.

Communicates effectively with other departments' staff concerning resident status.

Maintains confidentiality of residents and staff.

Responsible for completing and forwarding documentation for third party agencies i.e.: WCB, etc. as needed.

Participates in Family Conferences and Nursing Rounds engaging in collaborative practice to ensure best care outcomes for residents.

Provides supervision and direction for care routines for HCAs and LPNs.

Responsible for wound care management. Complete wound care management care plans. Ensure dressing supplies are re-ordered and resident wound care kits are assembled and maintained as per AHS standards in collaboration with Health Services Administrator

Supervises medication administration program and completes medication reconciliations as needed and monthly e-mar reviews.

Review care plans on a regular basis and notify Site RNs and AHS Case Managers to update as required.

Participate and promote quality improvement initiatives with RN Educators.

Assist in developing and maintaining new hire/orientation programs with the RN educators. Must meet Alberta Health & Wellness Continuing Health Services Standards.

Attends relevant committee meetings such as: Medication Safety, LPN Meetings, Occupational Health and Safety, Team and Nurses Meetings, Infection Control.

Participates in Care Conferences and In Service programs as a member of the Inter-disciplinary Team.

Acts in the role of Team Leader as assigned if/when DOC is temporarily absent from facility during the course of regular duties.

Complete other duties assigned

14 Day rotation with Saturday and Sunday off

The term of this temporary position may be extended.
