Duties
Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Administer medication and monitor vital signs in accordance with care plans and HIPAA regulations
Support clients with meal preparation, cooking nutritious meals, and ensuring proper nutrition
Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
Utilize patient observation skills to monitor health status, report changes, and document care activities accurately
Employ specialized techniques such as using a Hoyer lift or other assistive devices to safely transfer residents with mobility challenges
Assist with memory care needs including dementia and Alzheimer's care by implementing personalized strategies and maintaining a calm environment
Support residents in group home or assisted living settings by fostering social engagement and emotional well-being
Follow established care plans diligently while adapting to evolving needs of clients in home health or hospice settings
Maintain compliance with all safety protocols including CPR and First Aid certifications
Engage in ongoing communication with healthcare professionals, social workers, and family members to ensure holistic care delivery
Skills
Proven experience in home & community care environments or long-term care settings such as nursing homes or assisted living facilities
Strong knowledge of patient observation techniques, vital signs monitoring, and medication administration
Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, dementia, or memory impairments
Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
Excellent caregiving skills with a focus on compassionate patient support and direct assistance with ADLs
Knowledge of HIPAA regulations to ensure client confidentiality at all times
Experience working within care plans tailored for hospice or palliative care patients is a plus
Ability to handle multiple responsibilities efficiently while maintaining attention to detail in documentation and patient monitoring
Strong interpersonal skills for working effectively with clients from diverse backgrounds and their families
Physical stamina for long shifts involving standing, walking, lifting, and performing routine household tasks
